不同血液病患者血小板输注无效影响因素分析

摘　　要：目的:总结不同血液病患者血小板输注无效(PTR)的影响因素。方法:回顾性分析2013年10月-2018年7月广州市第一人民医院血液科住院患者随机输注机采血小板515例次的临床资料,利用血小板回收率(PPR)评价血小板输注效果,并分析影响PTR的因素。结果:血液病患者总PTR率为28.74%,再障(AA)、白血病、MDS、淋巴瘤、ITP、MM和其他患者的PTR率分别为:30.24%、22.78%、36.84%、36.84%、66.67%、11.11%和22.58%。与本院血液病患者总PTR率相比,只有ITP患者的PTR率明显增高(P<0.01)。再障患者中血小板抗体阳性和老年患者(≥60岁)的PTR率明显增高(P<0.01)。白血病患者中脾大和移植患者的PTR率明显增高(P<0.05)。结论:ITP患者PTR率明显高于其他血液病患者。PTR的影响因素因血液病的病种而异。Objective:To summarize the impact factors of platelet transfusion refractoriness(PTR)in patients with different hematologic diseases. Methods:The clinical data of 515 cases of platelet transfusion in patients with hematologic diseases in the hospital from Oct. 2013 to Jul. 2018 were analyzed retrospectively. Percentage platelet recovery(PPR)was used to evaluate the effect of platelet transfusion and analyze the cause of PTR in patients with hematologic diseases.Results:The PTR of patients with hematologic diseases in the hospital was 28.74%,the PTR rates in AA,Leukemia,MDS,Lymphoma,ITP,MM and other blood diseases were 30.24%,22.78%,36.84%,36.84%,66.67%,11.11% and22.58%.Platelet antibody positive and elderly patients(≥ 60 ages)had significantly higher PTR in patients with AA(all P<0.01). Splenomegaly and transplantation patients had significantly higher PTR in patients with Leukemia(all P<0.05). Conclusion:The PTR rate in patients with ITP was significantly higher than that in other patients with hematological diseases. There were influencing factors of PTR in patients with different hematologic diseases.
