摘 要:目的:明确妇幼保健医疗机构政策性亏损财政补偿存在的困境及其补偿需求的特殊性,探索可行的解决方案。方法:以北京市两家区级妇幼保健院为案例,通过成本测算,结合宏观医改政策环境,探讨政策性亏损的合理界定范围,提出适宜的财政补偿模式建议。结果:案例妇幼保健院在公共卫生服务人员经费和项目经费中均存在不同程度的亏损,现有的财政补偿模式无法满足妇幼保健公共卫生服务的需要。结论:建议以价格政策为导向,建立基于成本测算的动态补偿与监督的财政补偿机制,发挥财政在妇幼保健医疗机构经济运行中的引导和激励作用。Objective:To recognize the existing difficulties of financial compensation mechanism for policy losses in maternal and child health care institutions,explore the particularity for compensation demand,propose proper solution.Methods:Taking two Maternal and Child Health Hospitals of Beijing as an example,it calculates the cost of its public health services,analyzes the causes of financial compensation policy losses,calculates the data on policy-related losses and proposes suggestions.Results:Hospital has suffered losses of various degrees in both public health service personnel and project funds,but there is still suspicious of the rationality of loss.Conclusion:Guided by Price Policy,establishing a dynamic compensation and supervision mechanism based on cost measurement,the government finance should play the guiding and stimulating role of in the economic operation of public hospitals.
