摘 要:目的探讨临床药师基于疾病诊断相关分组(DRGs)数据参与消化内科药事精细化管理的实施效果,为促进临床合理用药提供参考。方法收集本院消化内科进入DRGs分组的出院患者作为研究对象。比较临床药师参与药事精细化管理干预前(2018年4~9月)、干预后(2019年4~9月)消化内科住院患者的基本情况及主要药事管理指标情况,并选取排名靠前的DRGs组进行入组例数、次均费用和药品费用等进行统计学分析,比较干预前后的费用差异。结果干预后,消化内科总权重、CMI值均明显升高,时间消耗指数、费用消耗指数、住院天数均有所降低;药品结构比、辅助药品结构比、次均药费、次均辅助药费均有所下降;DRGs为GU15、GK23、GK25组,干预后药品费用均较干预前有所下降,GK23组差异有统计学意义(P<0.01)。结论临床药师基于DRGs数据参与消化内科药事精细化管理的实践,在一定程度规范了本院消化内科的用药合理性,确保消化内科用药安全、有效、经济。Objective To determine the effect of clinical pharmacists participating in the precision management of medications in the Department of Gastroenterology in our hospital based on diagnosis related group(DRG)data,and to provide reference for rational drug use in the clinic.Methods The discharged patients who entered the DRGs in the Department of Gastroenterology were collected as research subjects.The basic situation of the inpatients in the Department of Gastroenterology and the main pharmacy management indicators before pharmacists participation(from April to September 2018)and after their intervention(from April to September 2019)were compared.The top-ranked DRGs were analyzed concerning the number of patients,costs each visit and drug costs,and the cost difference before and after the intervention.Results After the intervention,the total weight and CMI of the Department of Gastroenterology were significantly increased,and the time consumption index,cost consumption index,and hospitalization days were all reduced.The drug structure ratio,auxiliary drug structure ratio,drug cost each visit,and drug cost each visit were all reduced.There was a decrease in the drug costs in the GU15,GK23,and GK25 groups after the intervention,and significant difference in the GK23 group(P<0.001).Conclusion The participation of clinical pharmacists based on DRGs data helps regulate the clinical rational use of drugs and ensure the safety and effectiveness.
