奥马哈系统在关节镜下对前交叉韧带下止点撕脱骨折患者术后康复的影响

摘　　要：目的:探讨奥马哈系统在关节镜下对前交叉韧带下止点撕脱骨折患者术后康复的影响。方法:选取深圳市龙岗区骨科医院运动医学科2018年7月至2019年6月在关节镜下行前交叉韧带下止点撕脱骨折固定术的患者80例,采用随机抽样的方法分为对照组和观察组各40例。对照组按照常规的方法予以出院指导和随访;观察组应用奥马哈系统进行康复和护理指导。于术后随访比较:两组患者于术后第2、4、8、12周关节活动度的达标率;分析股四头肌萎缩指数于术前及术后第4、8、12周的异同点;术前、术后第12周两组患者Lysholm膝关节功能评分,用来评价康复效果。结果:术后第2、4、8、12周患侧膝关节活动度的达标率观察组均明显优于对照组,差异具有统计学意义(P<0.05),术前两组患者股四头肌萎缩指数比较,差异无统计学意义(P>0.05),观察组患者于术后第4、8、12周的股四头肌萎缩指数均低于对照组,差异具有统计学意义(P<0.05),两组患者膝关节Lysholm评分在术前比较,差异无统计学意义(P>0.05),术后第12周时,对照组Lysholm膝关节功能评分显著低于观察组,差异具有统计学意义(P<0.05)。结论:奥马哈系统护理模式可有效改善前交叉韧带下止点撕脱骨折固定术后患者存在的术后健康问题,促进膝关节功能恢复,提高患者术后膝关节活动度达标率,降低股四头肌萎缩指数。Objective To investigate the effect of Omaha system on the rehabilitation of patients with anterior cruciate ligament avulsion fracture under arthroscopy.Methods Eighty patients with anterior cruciate ligament avulsion fracture under arthroscopy from July 2018 to June 2019 in orthopaedic hospital of Shenzhen Longgang were selected.They were randomly divided into 40 cases,which including the control group and the observation group.The control group was given guidance and follow-up according to conventional methods;the observation group applied the Omaha system for rehabilitation and nursing guidance.Comparison of postoperative follow-up:the compliance rate of joint mobility in the two groups of patients at 2,4,8,and 12 weeks after surgery;analysis of the similarities and differences of the quadriceps atrophy index before and at 4,8,and 12 weeks after surgery;Lysholm knee joint function scores of two groups of patients before and after the 12th week were used to evaluate the rehabilitation effect.Results Before and after the 2,4,8,and 12 weeks,the compliance rate of the affected knee movement in the observation group was significantly shifted,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the quadriceps atrophy index of preoperative hypertension patients(P>0.05),In the observation group,the mean value of the quadriceps atrophy index decreased at 4th,8th,and 12th postoperatively,and the difference was statistically significant(P<0.05).The Lysholm score of the knee joint of dialysis patients was not significantly different before the operation(P>0.05).At 12 weeks before and after,except for Lysholm knee function score was significantly lower than the observation group,the difference was statistically significant(P<0.05).Conclusion Omaha system nursing mode can effectively improve the postoperative health problems of patients with anterior cruciate ligament bottom ankle avulsion fracture fixation,promote the recovery of knee function,increase the complianc
