不同年龄段受卵助孕患者体外受精-胚胎移植的临床结局比较

摘　　要：目的比较不同年龄段患者接受1次受卵助孕的体外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)的临床结局。方法回顾性分析2015年1月至2019年12月在郑州大学第二附属医院生殖医学中心接受1次受卵IVF及冷冻胚胎复苏移植的75例患者的临床资料,根据患者年龄的不同分为4组,<30岁组26例,30~<35岁组14例,35~<40岁组11例,≥40岁组24例。比较4组患者受卵数、优质胚胎数、冷冻胚胎数及复苏周期移植日内膜厚度、移植胚胎数、临床妊娠率、胚胎着床率、流产率等指标。结果①4组患者受卵周期的受卵数、优质胚胎数、冷冻胚胎数、复苏周期移植日内膜厚度、移植优质胚胎数、移植胚胎总数及临床妊娠率比较,差异均无统计学意义(P>0.05);②4组患者的胚胎着床率(年龄由低到高分别为67.65%、36.37%、40.00%、64.71%)、流产率(年龄由低到高分别为0、0、33.33%、35.71%)比较,差异有统计学意义(P<0.05)。结论不同年龄段受卵助孕患者的临床妊娠率相近,但35岁以上受卵助孕患者的流产率较高。Objective To compare the clinical outcomes of in vitro fertilization and embryo transfer(IVF-ET) in ovum donation recipients of different ages.Methods Retrospective analysis of the clinical data of 75 ovum donation recipients who received IVF and frozen embryo resuscitation transplantation in Second Affiliated Hospital of Zhengzhou University from January 2015 to December 2019.According to the age of the patients,they were divided into 4 groups,26 cases in the < 30 years old group,14 cases in the 30 ~ < 35 years old group,11 cases in the 35 ~ < 40 years old group,and 24 cases in the ≥40 years old group.The number of eggs received,the number of high-quality embryos,the number of frozen embryos,the thickness of endometrium on the day of transplantation,the number of transferred embryos,the clinical pregnancy rate,the rate of embryo implantation and the rate of abortion were compared among the four groups.Results ① There were no significant differences in the number of received eggs,high-quality embryos and frozen embryos,the thickness of endometrium on the day of transplantation,the number of transferred high-quality embryos,the total number of transferred embryos and the clinical pregnancy rate between the four groups(P>0.05) .② The difference of embryo implantation rate(67.65 %,36.37 %,40.00 %,64.71 %,age from low to high respectively) and abortion rate(0,0,33.33 %,35.71 %,age from low to high respectively) among the four groups was statistically significant(P<0.05).Conclusion The clinical pregnancy rate of different age groups is similar,but the abortion rate of patients over 35 years old is high.
