老年人心电图ST-T改变与TCD参数对比分析

摘　　要：目的探讨老年心电图ST-T改变患者经颅多普勒(TCD)参数异常的发生情况,为临床防治心脑血管疾病提供依据。方法80例行常规心电图及TCD检查的老年患者,根据心电图ST-T情况分为A组(心电图ST-T改变)和B组(心电图ST-T正常),各40例。两组患者均进行TCD检查。比较两组患者TCD参数异常发生率、伴随基础疾病情况。结果A组患者流速减慢发生率50.0%、搏动指数(PI)值升高发生率85.0%均高于B组的25.0%、62.5%,血流速度不对称发生率27.5%低于B组的50.0%,差异有统计学意义(P<0.05)。A组患者冠状动脉粥样硬化性心脏病(冠心病)发生率42.5%高于B组的15.0%,差异有统计学意义(P<0.05)。结论对伴有ST-T异常的老年患者应尽早行TCD检查评估脑血流情况,有助于心脑血管疾病早期诊治。Objective To discuss the occurrence of abnormal transcranial Doppler(TCD)parameters in the elderly patients with electrocardio gram ST-T changes,so as to provide basis for the treatment of cardiovascular and cerebrovascular diseases.Methods A total of 80 elderly patients undergoing routine electrocardiogram and TCD examination were divided into group A(changed electrocardiogram ST-T)and group B(normal electrocardiogram ST-T)according to electrocardiogram ST-T,with 40 cases in each group.Both groups received TCD examination.The incidence of TCD parameter abnormality and the condition of basic diseases were compared between the two groups.Results The incidence of slowing flow velocity 50.0%and incidence of increased pulsatility index(PI)85.0%in group A were higher than those in group B 25.0%and 62.5%,and incidence of asymmetric velocity 27.5%was lower than that in group B 50.0%,and the difference was statistically significant(P<0.05).The incidence of coronary heart disease 42.5%of group A was higher than that of group B 15.0%,and the difference was statistically significant(P<0.05).Conclusion TCD should be performed as early as possible in elderly patients with ST-T abnormalities to evaluate cerebral blood flow,which is helpful for the early diagnosis and treatment of cardiovascular and cerebrovascular diseases.
