温针刀治疗恢复期类风湿性膝关节强直临床观察

摘　　要：目的观察温针刀治疗恢复期类风湿性膝关节强直的临床疗效。方法选择2017年1月—2019年10月门诊收治的确诊为恢复期类风湿性膝关节强直的患者50例,给予温针刀治疗,每周2次,6次1个疗程。比较治疗前和治疗3周后的CRP、VAS评分、类风湿因子及晨僵积分,评估有效率。比较治疗前和治疗3个月后的膝关节功能改善情况。结果治疗3周后CRP、类风湿因子、VAS评分及晨僵积分均较治疗前降低,差异有统计学意义(P<0.05)。显效0例,有效47例,无效3例,总有效率为94%。治疗前优0例,良0例,可43例,差7例。3个月后优22例,良25例,可1例,差2例。治疗前CI:0.700~0.750,治疗3个月后CI:0.219~0.328。结论温针刀治疗恢复期类风湿性膝关节强直的临床疗效显著,费用低廉,能反复操作,值得临床推广。Objective To observe the clinical effect of warm needle knife in the treatment of rheumatoid knee ankylosis.Methods 50 patients diagnosed with rheumatoid knee ankylosis in convalescence from January 2017 to October 2019 were selected.They were treated with warm needle knife,twice a week,six times a course of treatment.The CRP,rheumatoid factor,VAS score and morning stiffness score before and 3 weeks after the treatment was compared,and the therapeutic effect was evaluated.The improvement of knee joint function before and 3 months after treatment was compared.Results After 3 weeks of treatment,CRP,rheumatoid factor,VAS score and morning stiffness score were lower than before treatment,and the difference was statistically significant(P<0.05).0 cases showed obvious effect,47 cases were improved,3 cases were ineffective,and the total effective rate was 94%.Before treatment,there were 0 excellent cases and 0 good cases,43 fair cases and 7 poor cases.3 months after treatment,there were 22 excellent cases,25 good cases,1 fair case and 2 poor cases.CI(0.70~0.75)was before treatment and CI was(0.219~0.328)3 months after treatment,and small value works well.Conclusion The warm needle knife in the treatment of rheumatoid knee ankylosis in convalescent has significant effect and low cost,and it can be operated repeatedly.It is worthy of clinical promotion.
