4链Kessler与4链十字缝合法修复手指屈肌腱的疗效对比

摘　　要：目的对比采用4链十字与4链Kessler缝合法修复指屈肌腱的疗效,并观察术后肌腱功能恢复情况。方法选择2017年6月-2019年6月行指屈肌腱修复术患者68例107指,分为对照组(4链Kessler缝合法)34例51指和观察组(4链十字缝合法)34例56指。术后6周、12周进行总主动活动度(TAM)和功能独立性评分(FIM)测定。结果术后第6周观察组TAM测定优于对照组(P=0.03),术后12周两组TAM测定无显著性差异(P=0.12);术后6周及12周FIM评分明显高于观察组(P<0.01)。结论手指屈肌腱损伤采用两种缝合方法后行早期功能训练,均能取得很好的临床疗效,但4链十字缝合法优于4链Kessler缝合法。Objective To observe the functional recovery of flexor tendon after operation by comparing4-chain cross with 4-chain Kessler suture.Methods A total of 107 fingers were selected from 68 patients undergoing flexor tendon repair in our hospital from June 2017 to June 2019.The patients were divided into the control group(4-chain Kessler suture),34 patients(51 fingers),and the observation group(4-chain cross suture),34 patients(56 fingers).Total active activity(TAM)and functional independence score(FIM)were measured at 6 and 12 weeks after surgery.Results The measurement of TAM in the observation group was better than that in the control group at the 6 th and 12 th weeks after the operation,but there was no statistical significance at the 12 th week after the operation(P=0.12),and the FIM score at the 6 th and 12 th weeks after the operation was significantly higher than that in the observation group(P<0.01).Conclusion Both two suture methods can achieve good clinical efficacy in early functional exercise,but 4-chain cross suture is better than 4-chain Kessler suture.
