掌骨克氏针固定与微型钢板固定在掌骨骨折治疗中的对照研究

摘　　要：目的探讨掌骨克氏针与微型钢板固定在掌骨骨折治疗中的效果与可行性。方法以2015年4月-2018年4月收治的70例掌骨骨折患者为研究对象,随机分为对照组(35例)和观察组(35例);对照组给予掌骨克氏针固定,观察组给予微型钢板固定;对比两组疗效、并发症及预后。结果观察组手术时间(45.7±5.6)min与对照组(44.8±6.4)min比较差异无统计学意义(P>0.05);观察组骨折愈合时间(69.2±8.3)d明显短于对照组(83.4±9.7)d(P<0.05);且其并发症发生率(8.6%)明显低于对照组(31.4%)(P<0.05)。术后12个月,观察组主动屈曲度优良率(94.3%)明显高于对照组(71.4%)(P<0.05)。结论微型钢板固定有助于缩短掌骨骨折手术时间,降低并发症发生率,促进患者恢复主动屈曲功能,值得临床推广应用。Objective To investigate the efficacy and feasibility of the metacarpal Kirschner wire and microplate fixation in the treatment of metacarpal fractures.Methods In our hospital from April 2015 to April 2018,70 patients with metacarpal fractures were randomly divided into the control group(35 cases)and the observation group(35 cases).The control group was given the metacarpal Kirschner wire fixation,and the observation group was given Mini-plate fixation;comparison of the two groups of efficacy,complications and prognosis.Results There was no significant difference between the operating time of the observation group(45.7±5.6 min)and the control group(44.8±6.4 min)(P<0.05);The time of fracture healing(69.2±8.3)d of the observation group were obviously shorter than the control group(7.7±3.5 d,83.4±9.7 d)(P<0.05).The incidence of complications of the observation group was 8.6%,and the control group was 31.4%,the difference was statistically significant(P<0.05).12 months after operation,the excellent rate of active buckling in the observation group was 94.3%,and the control group was 71.4%,the difference was statistically significant(P<0.05).Conclusion Miniplate fixation can shorten the length of operation time of patients with metacarpal fractures,reduce the incidence of complications,promote the recovery of active flexion.It is worthy of clinical application.
