摘　　要：目的探讨心肌梗死患者血清微小核糖核酸(miR)-208b、miR-302b表达水平,分析其与患者预后的相关性。方法选取自2017年11月至2019年11月收治的132例心肌梗死患者为研究对象。比较不同Killip分级、梗死部位患者血清miR-208b、miR-302b表达水平,分析血清miR-208b、miR-302b表达水平与心肌梗死患者临床特征的相关性。根据患者30 d预后情况将患者分为预后良好组(n=75)与预后不良组(n=57)。比较不同预后患者血清miR-208b、miR-302b表达水平。绘制受试者工作特征曲线,分析血清miR-208b、miR-302b表达水平预测心肌梗死患者30 d预后价值。以血清miR-208b、miR-302b表达量均数将患者划分为高表达者与低表达者,通过Kaplan-Meier曲线进行高低危生存分析。结果不同Killip分级患者血清miR-208b、miR-302b表达水平比较,差异有统计学意义(P<0.05)。不同梗死部位患者血清miR-208b、miR-302b表达水平比较,差异无统计学意义(P>0.05)。Spearman相关性分析显示,血清miR-208b、miR-302b表达水平与心肌梗死患者Killip分级呈明显正相关(r=0.498,r=0.512,P<0.05)。预后不良组患者血清miR-208b、miR-302b表达水平高于预后良好组,差异有统计学意义(P<0.05)。血清miR-208b预测心肌梗死患者30 d预后的受试者工作特征曲线下面积为0.759(95%可信区间0.677~0.829),当截断值>2.10时,敏感度为59.65%,特异度为88.00%。血清miR-302b预测心肌梗死患者30 d预后的受试者工作特征曲线下面积为0.793(95%可信区间0.714~0.859),当截断值>3.21时,敏感度为45.61%,特异度为97.33%。血清miR-208b、miR-302b联合预测心肌梗死患者30 d预后的受试者工作特征曲线下面积为0.864(95%可信区间0.793~0.917),敏感度为78.95%,特异度为80.00%。miR-208b、miR-302b高表达患者与低表达患者生存曲线比较,差异有统计学意义(χ^2=2.203,χ^2=3.405,P<0.01)。结论不同预后患者血清miR-208b、miR-302b水平存在明显差异,二者联�Objective To investigate the expression levels of microRNA(miR)-208b and miR-302b in serum of patients with myocardial infarction,and analyze their correlation with the prognosis of patients.Methods A retrospective study was performed on 132 cases of patients with myocardial infarction who were admitted from November 2017 to November 2019.The expression levels of serum miR-208b and miR-302b in patients with different Killip grades and infarction sites were compared,and the correlation between the expression levels of serum miR-208b and miR-302b and the clinical characteristics of patients with myocardial infarction was analyzed.Patients were divided into the group with good prognosis(n=75)and the group with poor prognosis(n=57)according to their prognosis at 30 days.The expression levels of serum miR-208b and miR-302b in patients with different prognosis were compared.The subject work characteristic curve was drawn,and the expression levels of serum miR-208b and miR-302b were analyzed to predict the prognostic value of patients with myocardial infarction for 30 days.The mean expression levels of serum miR-208b and miR-302b were used to divide the patients into high and low expression groups,and Kaplan-Meier curve was used for high-low risk survival analysis.Results Compared with the expression levels of serum miR-208b and miR-302b in patients with Killip classification,the difference was statistically significant(P<0.05).There was no statistically significant difference in the expression levels of miR-208b and miR-302b in patients with different infarcts(P>0.05).Spearman correlation analysis showed that the expression levels of serum miR-208b and miR-302b were positively correlated with Killip grading in patients with myocardial infarction(r=0.498,r=0.512,P<0.05).The expression levels of serum miR-208b and miR-302b in patients with poor prognosis were higher than those in patients with good prognosis(P<0.05).The area under the working characteristic curve of subjects whose serum miR-208b predicted the prognosis o
