摘  要:目的探讨小儿急性肺损伤患者血清的表面活性蛋白(SP)-D、SP-A表达情况,并分析其与LIS的相关性。方法选择2019年7月至2020年1月重庆医科大学附属第一医院大足医院接诊的65例急性肺损伤儿童为研究对象,设为观察组。其中伴有脏器功能衰竭34例,无脏器功能衰竭31例。另选择同期在我院体检的55例儿童作为体检组。分析血清SP-D、SP-A、LIS水平变化情况,及其相关性分析。结果观察组患儿血清SP-D、LIS评分显著高于体检组,SP-A水平显著低于体检组,差异均有统计学意义(P<0.05)。有脏器功能衰竭患儿SP-D、LIS水平显著高于无脏器功能衰竭患儿,SP-A水平显著低于无脏器功能衰竭患儿,差异均有统计学意义(P<0.05)。将LIS作为因变量,将血清SP-D、SP-A分别作为自变量,SP-D和LIS均呈正相关(r=0.709,P<0.05),SP-A和LIS呈负相关(r=-0.828,P<0.05)。结论急性肺损伤患儿的SP-D、SP-A与LIS的表达和病情严重程度之间存在着密切关系,SP-D和LIS均呈正相关,SP-A和LIS呈负相关,可为靶向药物治疗小儿急性肺损伤提供新思路。Objective To investigate the expression of surfactant protein(SP)-D and SP-A in serum of children with acute lung injury,and to analyze their correlation with lung injury score(LIS).Methods A retrospective study was performed on 65 cases of children with acute lung injury who were admitted from July 2019 to January 2020(observation group),including 34 cases with organ failure and 31 cases without organ failure.In addition,55 children who underwent physical examination at the same time were selected as the physical examination group,the changes of serum surfactant protein(SP)-D,SP-A and LIS levels and the correlation between them was analyzed.Results The SP-D and LIS scores of the observation group were significantly higher than those of the physical examination group,and the SP-A level was significantly lower than that of the physical examination group,with statistically significant differences(P<0.05).The levels of SP-D and LIS in children with organ failure were significantly higher than those in children without organ failure,and the levels of SP-A were significantly lower than those in children without organ failure(P<0.05).With LIS as the dependent variable and SP-D and SP-A as the independent variables,SP-D and LIS were positively correlated(r=0.709,P<0.05),while SP-A and LIS were negatively correlated(r=-0.828,P<0.05).Conclusion SP-D and SP-A are closely related to the expression and severity of LIS in children with acute lung injury.SP-D and LIS are positively correlated,while SP-A and LIS are negatively correlated,which may provide a new idea for targeted drug therapy for children with acute lung injury.
