带部分甲床甲襞的拇趾腓侧皮瓣急诊修复手指远节指腹软组织合并部分甲床缺损伤的临床应用

摘　　要：目的探讨采用带部分甲床甲襞的拇趾腓侧皮瓣急诊修复手指远节指腹软组织合并部分甲床缺损伤的临床应用。方法2016年1月-2019年10月,对15例手指远节指腹软组织并部分甲床甲襞缺损伤患者采用带部分甲床甲襞的拇趾腓侧皮瓣进行急诊手术修复。结果15例皮瓣均成活,术后随访6~22个月(平均12个月),统计患者对皮瓣满意度、感觉及功能。术后满意度为87%(13/15,其中2例受区指甲有纵线),皮瓣两点辨别觉为3~6 mm,手指功能恢复满意:结果为优12例,良3例。供区足趾创面甲级愈合,行走功能未见异常,外观未见明显畸形。结论带部分甲床甲襞的拇趾腓侧皮瓣能同时修复手指远节指腹软组织合并部分甲床缺损伤,手术效果满意,值得在临床上推荐使用。Objective To explore the clinical application of emergency surgical to repair the soft tissue of finger-pulp and part of nail bed defects of distal part of fingers by using the fibular flaps of hallux with part of nail bed and nail fold.Methods From January,2016 to October,2019,15 cases of patients,with soft tissue of finger-pulp and part of nail bed defects of distal part of fingers were repaired by the fibular flaps of hallux with part of nail bed and nail fold.Results All flaps survived and the satisfaction of patients was 87%(13/15,of which 2 cases had longitudinal lines on the nails),and the two-point discrimination of the flap was 3~6 mm.The satisfaction survey of finger function recovery showed that there were 12 cases with excellent results and 3 cases with good results.Conclusion The effects of this surgical method were satisfactory and is a reliable method.
