脉冲染料激光联合曲安奈德治疗甲状腺术后增生性瘢痕的疗效

摘　　要：目的探讨脉冲染料激光联合曲安奈德治疗甲状腺术后增生性瘢痕的疗效。方法2016年8月至2017年10月,将威海市立医院皮肤医疗美容科甲状腺术后增生性瘢痕患者76例(男22例,女54例,年龄18~48岁,平均31岁)分为两组。观察组38例采用595 nm脉冲染料激光联合曲安奈德注射液局部注射;对照组38例仅局部注射曲安奈德。每4周1次,未痊愈及复发者继续治疗,共治疗5次判断疗效和不良反应,随访12个月。结果治疗后观察组治愈21例,占55.26%;有效32例,占84.21%;对照组治愈4例,占10.53%,有效16例,占42.11%,观察组治愈率及有效率均高于对照组(χ^2=5.371,4.989,P<0.01)。结论595 nm脉冲染料激光联合局部注射曲安奈德治疗甲状腺术后增生性瘢痕见效快,不良反应小,疗效确切。Objective To investigate the effect of pulsed dye laser combined with triamcinolone acetonide on hypertrophic scar after thyroid surgery.Methods From August 2016 to October 2017,76 patients(22 males and 54 females,aged between 18 and 48 years,with an average of 31 years)with hypertrophic scar after thyroid surgery in the Department of Dermatology and Cosmetology,Weihai City Hospital,Shandong Province,were divided into two groups.In the observation group,38 patients were subjected to 595 nm pulsed dye laser combined with triamcinolone acetonide injection.In the control group,38 patients were only locally injected with triamcinolone acetonide.Once every 4 weeks,the patients who did not recover or had a recurrence continued treatment,and the efficacy and adverse reactions were judged for 5 times in total.The patients were followed up for 1 year.Results After treatment,21 cases(55.26%)were cured in the observation group.32 cases were effective,accounting for 84.21%.4 patients in the control group were cured,accounting for 10.53%,and 16 patients were effective,accounting for 42.11%.The cure rate and effective rate in the observation group were higher than those in the control group(χ^2=5.371,4.989,P<0.01).Conclusions 595 nm pulsed dye laser combined with local triamcinolone acetonide injection is effective in the treatment of postoperative hypertrophic scar.
