摘 要:目的:研究用椎弓根螺钉内固定术联合骨水泥椎体成形术治疗胸腰椎压缩性骨折的临床效果。方法:将2019年2月至2020年2月广东省中山市中医院收治的75例胸腰椎压缩性骨折患者随机分为联合手术组(CO组,n=38)与椎弓根螺钉内固定术组(PSF组,n=37)。用椎弓根螺钉内固定术对PSF组患者进行治疗,用椎弓根螺钉内固定术联合骨水泥椎体成形术对CO组患者进行治疗,然后比较两组患者的美国脊髓损伤协会(ASIA)评分、脊柱的高度、脊柱的Cobb角及治疗后其脊髓损伤的恢复情况。结果:治疗后,CO组患者ASIA评分中的触觉评分和运动评分均高于PSF组患者,差异有统计学意义,P<0.05。治疗后,CO组患者脊柱的高度高于PSF组患者,其脊柱的Cobb角大于PSF组患者,差异有统计学意义,P<0.05。治疗后,CO组患者脊髓损伤的总恢复率高于PSF组患者,差异有统计学意义,P<0.05。结论:用椎弓根螺钉内固定术联合骨水泥椎体成形术治疗胸腰椎压缩性骨折能有效地恢复患者脊柱的高度和Cobb角,减轻其脊髓损伤,改善其运动功能。Objective:To study the clinical effect of pedicle screw fixation combined with bone cement vertebroplasty in the treatment of thoracolumbar compression fractures.Methods:75 patients with thoracolumbar compression fractures admitted to Zhongshan Hospital of Traditional Chinese Medicine,Guangdong Province from February 2019 to February 2020 were randomly divided into a combined operation group(CO group,n=38)and a pedicle screw internal fixation group(PSF group,n=37).Patients in the PSF group were treated with pedicle screw fixation and patients in the CO group were treated with pedicle screw fixation combined with bone cement vertebra arthroplasty.Then the ASIA score,spine height,Cobb Angle of the spine,and post-treatment recovery from spinal cord injury were compared between the two groups.Results:After treatment,the ASIA scores of PATIENTS in THE CO group were higher than those in the PSF group,with statistically significant difference(P<0.05).After treatment,the height of the spine in THE CO group was higher than that in the PSF group,and the Cobb Angle of the spine was higher than that in the PSF group,with a statistically significant difference(P<0.05).After treatment,the total recovery rate of spinal cord injury in THE CO group was higher than that in the PSF group,with a statistically significant difference(P<0.05).Conclusion:Pedicle screw fixation combined with bone cement vertebroplasty in the treatment of thoracolumbar compression fractures can effectively restore the height and Cobb Angle of the spine,reduce spinal cord injury and improve motor function.
