不同剂量肝素钠封管液对肝硬化失代偿期患者出凝血的影响

摘　　要：目的探究不同剂量肝素钠封管液对肝硬化失代偿期患者出血凝血的影响。方法将2017年3月至2018年11月我院收治的肝硬化失代偿期患者73例,随机分为A组(18例)、B组(18例)、C组(18例)、D组(19例),分别予以62.50 U/mL肝素钠2.00、3.00、4.00、5.00mL,比较4组出血倾向、静脉炎发生率、堵管发生率、静脉留置针留置时间。结果4组患者穿刺前、拔针后血小板计数、凝血酶原时间、凝血活酶时间,差异无统计学差异(P>0.05),D组患者堵管率5.30%与A组33.33%相比,差异有统计学差异(P<0.05);与A组、C组、D组相比,B组留置时间具有统计学差异(P<0.05)。结论在对肝硬化失代偿期患者静脉留置针肝素封管液剂量选择上,以3.00 mL的62.50 U/mL肝素钠为最优剂量。Objective To investigate the effect of different doses of heparin sodium sealing fluid on bleeding and coagulation in patients with decompensated cirrhosis.Methods In March 2017 to November 2018,our hospital patients with cirrhosis of the liver decompensation period of 73 cases were randomly divided into group A(18 cases)and group B(18 cases),group C(18 cases),group D(19 cases),respectively,to 62.50 U/mL heparin sodium 2.00,3.00,4.00,5.00 mL,compare four groups of bleeding tendency,the incidence of phlebitis,incidence of blocking pipe,venous indwelling needle indwelling time.Results There was no significant difference in platelet count,prothrombin time,and thromboplastin time before puncture and after needle removal in the 4 groups(P>0.05).The blocking rate of patients in group D was 5.30%compared with 33.33%in group A.There was a statistical difference(P<0.05);compared with group A,group C,and group D,the indwelling time of group B was statistically different(P<0.05).Conclusion The optimal dose of 3.00 mL of 62.50 U/mL heparin sodium was selected for intravenous indwresting heparin sealing solution for patients with decompensated cirrhosis.
