踝关节牵张术联合PRP关节内注射治疗青年创伤性踝关节炎的临床研究

摘　　要：目的 探究踝关节牵张术联合富血小板血浆(PRP)关节内注射治疗青年创伤性踝关节炎(TAA)的临床疗效.方法 选取我院收治的86例TAA青年患者列为研究对象,按随机数字表法以1:1比例分为对照组与研究组,每组各43例.对照组患者使用玻璃酸钠关节内注射进行治疗,研究组患者使用踝关节牵张术+PRP进行治疗.对比两组患者治疗前及治疗3个月后疼痛程度(VAS)、肿胀程度、踝关节功能评分(Baird-Jackson踝关节评分),踝关节活动度(ROM),并记录并发症发生情况.结果 治疗3个月后,两组患者VAS、肿胀程度显著下降而ROM和Baird-Jackson评分显著上升(P<0.05),且研究组患者上述指标与同期对照组对比差异显著(P<0.05);治疗3个月后,两组患者并发症发生率对比无差异(P>0.05).结论 应用踝关节牵张术与PRP关节内注射结合治疗TAA青年患者,可显著缓解患者疼痛及肿胀,促进恢复踝关节功能,且并发症发生率较小,值得推广.Objective To explore the clinical efficacy of ankle joint distraction combined with intraarticular injection of platelet-rich plasma(PRP)in the treatment of traumatic ankle arthritis(TAA)in youth.Methods Eighty-six TAA patients admitted to our hospital from April 2016 to April 2018 were selected as the study subjects.The patients were evenly divided into control group and study group by random number table method,with 43 cases in each group.Patients in control group were treated with intraarticular injection of sodium hyaluronate while patients in study group were treated with ankle joint distraction+PRP.The pain degree(VAS),swelling degree,ankle joint function score(Baird-Jackson ankle joint score)and ankle joint range of motion(ROM)were compared between the two groups before treatment and after 3 months of treatment,and the occurrence of complications were recorded.Results After 3 months of treatment,the VAS degree and swelling degree in the two groups were significantly decreased while the ROM and Baird-Jackson score were significantly increased(P<0.05),and the above indexes in study group were significantly different from those in control group(P<0.05).After 3 months of treatment,there was no difference in the incidence rate of complications between the two groups(P>0.05).Conclusion Ankle joint distraction combined with intraarticular injection of PRP for TAA young patients can significantly alleviate the pain and swelling,and promote the recovery of ankle joint function,and it has low incidence rate of complications,thus it is worth promoting.
