摘　　要：目的探讨沙美特罗-丙酸氟替卡松联合噻托溴铵干粉剂治疗老年稳定期重度慢性阻塞性肺疾病(COPD)的疗效及对肺功能的影响。方法采取随机双盲研究法,将2016年1月~2018年1月南通市海安县中医院收治的老年稳定期重度COPD患者116例,按照随机数字表均分为两组。对照组行沙美特罗-丙酸氟替卡松吸入,观察组在此基础上联合噻托溴铵干粉剂。对比治疗前、治疗3个月后临床疗效、6 min步行距离、肺功能水平(FEV 1、FVC、FEV 1%pre)及不良反应发生率。结果治疗3个月后,与对照组相比,观察组治疗总有效率明显增高、6min步行距离明显延长、肺功能水平(FEV 1、FVC、FEV 1%pre)明显提高、不良反应发生率明显降低,差异均有统计学意义(P<0.05)。结论沙美特罗-丙酸氟替卡松联合噻托溴铵干粉剂治疗老年稳定期重度COPD可明显提高临床疗效,改善肺功能,减轻不良反应,值得推广。Objective To investigate the effect of salmeterol/fluticasone propionate combined with tiotropium bromide in the treatment of older patients with stable severe chronic obstructive pulmonary disease(COPD)and the effect on lung function.Methods 116 older patients with stable severe COPD in Hai’an Hospital of Traditional Chinese Medicine were randomly divided into the control group and the observation group according to random number table from January 2016 to January 2018.The control group was treated with salmeterol/fluticasone propionate,and the observation group was treated with salmeterol/fluticasone propionate and tiotropium bromide.The clinical effect,6-minute walking distance,pulmonary function(FEV 1,FVC,FEV 1%pre)and the incidence of adverse reactions were compared between the two groups before treatment and after treatment for 3 months.Results 3 months after treatment,the overall response rate was higher,the 6-minute walking distance was longer,the indexes of FEV 1,FVC and FEV 1%pre were higher,and the incidence of adverse reactions was lower in the observation group than those in the control group,there were significant differences between the two groups(P<0.05).Conclusion Combination of salmeterol/fluticasone propionate and tiotropium bromide might improve the clinical efficacy and lung function,and reduce the side effects in the treatment of stable severe COPD in older patients.
