大型综合性医院多院区COVID-19院感防控策略探讨

摘　　要：目的探讨大型综合性医院多院区COVID-19防控策略。方法以某苏中地区大型三甲综合性医院三个院区为例,根据疫情变化和医院自身情况,不断调整院感防控策略,最终以其中一个院区作为专门收治COVID-19患者的传染病院区,打造泰州版的"小汤山"模式,探讨取得的效果。结果改造后总院和南院的潜在传播风险值降低到1分和3分,3个院区早期和后期存在的传播风险差异有统计学意义(P<0.05);后期3个院区人力资源岗位数由43个下降到27个,差异有统计学意义(P<0.05);二级防护后期人力岗位数较前期3个院区共减少8个,三级防护后期3个院区共减少8个。目前,医院无工作人员院内感染,无其他患者和家属感染。结论作为定点收治新冠肺炎的医疗机构,在大型突发应急事件中迅速调整多院区布局,分区把控重点防御收治,打造出大型综合医院多院区COVID-19院感防控的"小汤山"模式,以达到医务人员零感染、人际间零传染的目的。通过阻断医患、医护、护患的传播途径的方式,为今后紧急状态下迅速改建独立院区、应对突发呼吸系统疾病与传染病收治需求提供借鉴。Objective To explore the COVID-19 prevention and control strategies of COVID-19 by large comprehensive hospitals.Methods The three branches of a general hospital in the middle of Jiangsu province were engaged in the study.Based on the changes in the epidemic situation and its own conditions,the hospital continuously adjusted prevention and control strategies.In the end,its southern branch was designated as a hospital specialized in the treatment and prevention of COVID-19,creating a Taizhou version of the"Xiaotangshan"model for the purpose of probing into the prevention and treatment effects.Results After the renovation,the potential infection risk of its headquarter and Southern Branch was reduced to 1 and 3 points respectively,and the infection risk in the early and later period significantly differed across the three branches statistically(P<0.05).In the later stage,the number of posts in human resources departments of three branches significantly decreased from 43 to 27(P<0.05).The number of posts in the later stage during second-level protection and third-level protection periods was reduced by 8 respectively as compared of the previous stage.There was no report of in-hospital infection cases from the faculty,patients and their members.Conclusion As a designated medical institution for the treatment of COVID-19,it quickly adjusted the layout of multiple branches in the emergency and focused on the prevention and treatment in the branches,creating the"Xiaotangshan"mode of COVID-19 hospital infection prevention and control in its branches.As a result,it achieved zero infection among the medical staff.Its strategy of blocking the transmission routes between doctors and patients,doctors and nurses,nurses and patients provides a good example for the rapid reconstruction of separate hospital districts,the treatment of respiratory emergencies and infectious diseases in the future emergency.
