摘 要:目的观察异氟烷(isoflurane,IF)吸入麻醉对脑缺血再灌注大鼠海马区c-fos和Bax表达及认知功能的影响。方法2月龄雄性Wistar大鼠32只,体重180~220 g,随机分为四组(n=8):假手术组(Sham),缺血再灌注模型组(IR),IR+IF(1.5%)组,IR+IF(2.0%)组。使用线栓法缺血2 h后,行再灌注处理,制备脑缺血再灌注损伤模型。麻醉组分别吸入1.5%、2.0%的异氟烷。再灌注4 h后,NSS法测试大鼠神经功能,干湿重法检测大鼠脑组织含水量,比色法检测各组大鼠脑组织中SOD活性和MDA含量,qRT-PCR和Western blot检测各组大鼠海马区c-fos、Bax表达情况。结果异氟烷麻醉组神经行为学评分、脑组织含水量、MDA含量显著低于模型组,SOD活性高于模型组(P<0.05);异氟烷(2.0%)组大鼠海马区c-fos、Bax表达较模型组减弱(P<0.05),且明显少于异氟烷(1.5%)组。结论异氟烷对缺血神经元有一定保护作用,能减弱c-fos、Bax的表达。Objective To investigate the effects of inhalation anesthesia with isoflurane(IF)on the expressions of c-fos and Bax in hippocampus and cognitive function of rats with cerebral ischemia/reperfusion injury.Methods Thirty-two 2-month-old male Wistar rats,weighing 180-220 g,were randomly divided into four groups(n=8):sham group,ischemia/reperfusion(I/R)group,I/R+1.5%isoflurane anesthesia groupand I/R+2.0%isoflurane anesthesia group.After 2 hours of right middle cerebral artery occlusion by the method of thread embolization,reperfusion treatment was performed to establish the model of cerebral ischemia reperfusion injury.In the I/R+1.5%isoflurane anesthesia group and I/R+2.0%isoflurane anesthesia group,rats with cerebral ischemia/reperfusion injury inhaled 1.5%and 2.0%isoflurane respectively.Four hours after reperfusion,the neurological function of rats was scored by NSS method.The water content in brain tissue was measured by dry-wet weight method and the activity of SOD and the content of MDA were detected by colorimetry.RT-q PCR and Western blot method were performed to detect the expressions of c-fos and Bax in hippocampus zone.Results The neurobehavioral score,brain water content and MDA content in the isoflurane anesthesia group were significantly lower than those in the I/R groupand SOD activity was higher than that in I/R group.The levels of c-fos and Bax were significantly reduced in the 2.0%isoflurane anesthesia group,which was significantly lower than those in the 1.5%isoflurane anesthesia group.Conclusion Isoflurane has protective effects on ischemic neurons,and can downregulate the expression of c-fos and Bax.
