摘  要:目的探讨产前超声诊断胎儿冠状动脉代偿性扩张(CACD)的临床价值。方法回顾性分析经出生后超声心动图检查或引产后尸检证实的17胎CACD胎儿,观察其产前超声声像图特征,随访妊娠结局。结果17胎CACD左右CA均受累,15胎于晚孕期、2胎于中孕期获得诊断,均伴不同程度血流动力学异常;其中10胎存在不同程度水肿,7胎伴Galen静脉瘤;8胎生长发育受限;8胎伴结构畸形。17胎中,8胎引产,3胎胎死宫内;6胎经紧急剖宫产出生,其中2例早产儿于出生1周内夭折,4名为健康足月儿。结论产前超声可诊断胎儿CACD,并及时预警胎儿宫内缺氧。Objective To explore the value of prenatal ultrasound in diagnosis of coronary artery compensatory dilatation(CACD).Methods Data of 17 CACD fetuses confirmed with echocardiography after birth or autopsy after labor induction were retrospectively analyzed.The ultrasonic features were observed,and the pregnancy outcomes were followed up.Results CACD involved left and right CA in all 17 fetuses,15 were diagnosed in late pregnancy and 2 in middle pregnancy,all accompanied with hemodynamic abnormalities,including 10 with different degree fetal edema and 7 with Galen venous aneurysm.Fetal growth restriction and other structural abnormalities were observed each in 8 fetuses.Among 17 fetuses,induced labor was performed in 8,intrauterine fetal death occurred in 3,whereas 6 born through emergency cesarean section,including 2 premature died within 1 week after birth and 4 full-term healthy newborns.Conclusion Prenatal ultrasound can be used to diagnose fetal CACD and early warning fetal intrauterine hypoxia.
