摘 要:[目的]探讨一次性采血针与静脉留置针采血导致溶血及对检验结果的影响。[方法]计算机检索PubMed、ScienceDirect、Google academic、中国知网、万方数据库和维普数据库中涉及一次性采血针与静脉留置针采血导致溶血及对检验结果影响的随机对照试验,检索时间为2012年1月1日—2019年5月31日。文献筛选、数据提取和质量评价后,使用Review Manager 5.3软件进行分析。[结果]共纳入21篇文献。Meta分析结果显示:留置针与一次性采血针对血小板计数[MD=4.25,95%CI(0.76,7.75),P=0.02]影响差异有统计学意义,留置针与一次性采血针对标本溶血[OR=1.72,95%CI(0.87,3.41),P=0.12]、血清钾[MD=0.02,95%CI(-0.16,0.20),P=0.86]、血清钠[MD=0.05,95%CI(-0.86,0.97),P=0.91]、血清钙[MD=0.00,95%CI(-0.05,0.05),P=0.95]、血清氯[MD=-0.44,95%CI(-1.61,0.74),P=0.47]、白细胞计数[SMD=-0.41,95%CI(-1.23,0.41),P=0.32]、红细胞计数[SMD=0.35,95%CI(-0.44,1.14),P=0.38]、血红蛋白[SMD=-0.23,95%CI(-0.66,0.19),P=0.28]、葡萄糖[SMD=-0.11,95%CI(-0.34,0.13),P=0.38]影响差异无统计学意义。[结论]现有证据表明留置针采血与一次性采血针采血,除外周血血小板计数以外,均可得到可靠一致的检验结果,且标本溶血率无差异。Objective:To explore hemolysis caused by disposable blood collection needle and venous indwelling needle blood collection and their effects on test results.Methods:Randomized controlled trials on hemolysis caused by disposable blood collection needle and venous indwelling needle blood collection and their effects on test results,were retrieved from PubMed、ScienceDirect、Google academic,CNKI,Wanfang Data and VIP.The retrieval time was from January 1,2012 to May 31,2019.After literature screening,data extraction and quality evaluation,Review Manager 5.3 software was used for analysis.Results:A total of 21 articles were included.Meta analysis results showed that there were a statistically significant difference on platelet count between indwelling needle and disposable blood collection[MD=4.25,95%CI(0.76,7.75),P=0.02].However,there were no statistical difference on hemolysis[OR=1.72,95%CI(0.87,3.41),P=0.12]serum potassium[MD=0.02,95%CI(-0.16,0.20),P=0.86],serum sodium[MD=0.05,95%CI(-0.86,0.97),P=0.91],serum calcium[MD=0.00,95%CI(-0.05,0.05),P=0.95],serum chlorine[MD=-0.44,95%CI(-1.61,0.74),P=0.47],white blood cell count[SMD=-0.41,95%CI(-1.23,0.41),P=0.32],red blood cell count[SMD=0.35,95%CI(-0.44,1.14),P=0.38],hemoglobin[SMD=-0.23,95%CI(-0.66,0.19),P=0.28],glucose[SMD=-0.11,95%CI(-0.34,0.13),P=0.38]between two blood collection methods.Conclusions:Existing evidence shows that except for peripheral blood platelet count,indwelling needle blood collection and disposable blood collection can both obtain reliable and consistent test results.And there is no difference on hemolysis rate of hemolysis.
