负压封闭引流联合次氯酸冲洗治疗慢性创面的效果研究

摘　　要：[目的]探讨负压封闭引流(VSD)联合次氯酸冲洗治疗慢性创面的临床疗效。[方法]选取2018年2月—2018年12月我院收治的慢性创面病人84例,根据信封法随机分为试验组和对照组,剔除退出病例,试验组和对照组最终纳入有效病例各40例。对照组连续7 d行VSD治疗,试验组连续7 d在VSD基础上联合次氯酸冲洗,每日2次,每次30 min。比较两组病人肉芽组织新生毛细血管生长和纤维蛋白分布情况、引流管堵塞发生率、泡沫干瘪率、细菌清除率、肉芽组织覆盖率、不良反应发生率。[结果]治疗后7 d,试验组创面情况和组织病理学观察结果优于对照组;试验组引流管堵塞发生率、泡沫干瘪率低于对照组(P<0.05);试验组细菌清除率、肉芽组织覆盖率高于对照组(P<0.05);两组不良反应发生率差异无统计学意义(P>0.05)。[结论]与单独使用VSD治疗相比,VSD联合次氯酸冲洗能降低引流管堵塞率和泡沫干瘪率,有效控制慢性创面感染,促进毛细血管和肉芽组织生长。Objective:To investigate clinical effect of vacuum sealing drainage(VSD)combined with hypochlorous acid irrigation in treatment of chronic wounds.Methods:A total of 84 patients with chronic wounds admitted in our hospital were randomly divided into intervention group and control group by envelope method from February 2018 to December 2018.There were 40 effective cases both in intervention group and control group after withdrawal cases were excluded.The patients in control group were treated with VSD for 7 days.The patients in intervention group were treated with VSD combined with hypochlorous acid irrigated for 7 days,twice a day for 30 minutes each time.Capillary growth of granulation tissue and fibrin distribution,incidence of drainage tube blockage,rate of foam collapse,rate of bacterial clearance,coverage of granulation tissue,and incidence of adverse reactions were compared between two groups.Results:After 7 days of treatment,capillary growth of granulation tissue and fibrin distribution in intervention group were better than that in control group.Incidence of drainage tube blockage and rate of foam collapse in intervention group were lower than that in control group(P<0.05).Rate of bacterial clearance and coverage of granulation tissue in intervention group were higher than that in control group(P<0.05).There was no statistically significant difference in incidence of adverse reactions between two groups(P>0.05).Conclusions:Compared with VSD treatment alone,VSD combined with hypochlorous acid irrigation can reduce rate of drainage tube blockage and foam collapse,effectively control chronic wound infection,and promote growth of capillary and granulation tissue.
