摘 要:目的调查不同妊娠期、不同年龄孕妇体内维生素E(VE)营养状态,探讨α-生育酚作为评价指标的适用性。方法回顾性研究,选取2016年10月至2017年10月期间在北京积水潭医院回龙观院区产科规律产检的孕妇,1 840例孕妇均遵医嘱在妊娠早、中、晚期进行血液采集,采用高效液相色谱法(HPLC)检测血清中α-生育酚水平。结果α-生育酚水平随孕周增加而增加(F=6 409.353,P<0.001),同时VE增多率也由妊娠早期的0.33%、妊娠中期的9.51%,增至妊娠晚期的34.18%;在妊娠早、中期,孕妇年龄越大α-生育酚水平越高(F=11.376、P<0.001,F=6.282,P<0.001),在妊娠晚期虽然差别不具有统计学意义(F=2.343、P=0.071),但是随着孕妇怀孕年龄的增大,α-生育酚水平有增高的趋势。结论北京市昌平地区的孕妇在妊娠期间VE水平按目前的分层标准,极少存在缺乏的表现,异常主要表现为VE增多。α-生育酚水平随孕周增加而增加,怀孕年龄越大α-生育酚水平越高。但是在分析临床检测数值时,应该充分考虑年龄、特别是妊娠期间血脂生理性增多对循环中α-生育酚水平的影响,审慎做出VE过剩的结论。Objective To investigate the nutritional status of vitamin E in pregnant women with different gestation stages and ages, and to explore the applicability of α-tocopherol as an evaluation index.Methods 1 840 pregnant women who underwent regular prenatal examinations in Huilongguan branch of Beijing Jishuitan hospital from October, 2016 to October, 2017 were selected for the study. Blood samples were collected in the early, middle and late trimester of pregnancy as prescribed by clinician, and serum levels of α-tocopherol were detected by HPLC.Results The level of α-tocopherol increased with gestational age(F=6 409.353,P<0.001). Meanwhile, the rate of vitamin E excess increased from 0.33% in the first trimester and 9.51% in the second trimester further to 34.18% in the third trimester. In the early and middle trimesters of pregnancy, the older the woman were, the higher the level of α-tocopherol(F=11.376、P<0.001,F=6.282,P<0.001)was. Although the difference in the third trimester of pregnancy was not statistically significant(F=2.343,P=0.071), the level of α-tocopherol increased with the increase of age.Conclusion According to the current stratification standard, the abnormal performance of vitamin E among pregnant women during pregnancy in Changping district of Beijing is mainly manifested as an increase trend, and the deficiency is rare. The level of α-tocopherol increases with the gestational age, and the higher the age is, the higher the α-tocopherol level is. However, for clinician′s analysis, the influence of age, especially the physiological increase of serum lipids during pregnancy on the concentration of α-tocopherol in circulation, should be fully taken into account, and the conclusion of vitamin E excess should be made prudently.
