公立医院临床路径应用现状及效果评价--以云南省某公立医院腰椎间孔镜手术为例

摘　　要：为进一步探讨临床路径在公立医院管理中的应用效果,在简要介绍临床路径应用现状的基础上,以云南省某公立医院椎间孔镜手术实施临床路径管理为例,对比实施临床路径管理前后平均住院费用、住院时间和患者满意度等指标,结果显示:应用临床路径可明显提高医疗质量及手术安全性、缩减患者住院天数、降低住院费用、提高患者满意度,可提高工作效率,节约医疗成本。但在应用中也存在受医保支付制度限制、相关软件开发滞后、医务人员对其概念理解存在偏差等问题,应加强医院临床路径相关系统的建设及完善,加强医务人员相关知识及技能的培训,更好地让临床路径管理服务于临床实践中。For further discussing the application effect of clinical pathway in public hospital management,based on brief introduction of clinical pathway application,it takes lumbar foramina surgery in a public hospital as example,compares index of average hospitalization expenses,hospital stays,patients satisfaction and so on before and after the implementation of clinical pathway management.The results show that the application of clinical pathway may obviously improve medical quality and operation safety,curtail patients hospital stays,reduce hospitalization expenses,improve patients satisfaction,enhance work efficiency and save medical cost.But it is limited by medical insurance payment system in the application,hysteretic relevant software development,the deviation of medical staffs understanding to its concept.It should strengthen the construction and perfection of clinical hospital pathway relevant system,strengthen relevant knowledge and skill training for medical staffs,so as to clinical pathway management can further serve clinical practice.
