摘 要:骨不愈合是骨科常见且棘手的并发症之一,其不仅延长治疗周期,加重家庭及社会负担,甚至会诱发畸形、残疾等严重后果。而基于牵张成骨的手风琴技术在对该病的治疗中疗效显著。研究发现其牵拉、压力及轴向微动等应力作用会刺激调控血管内皮细胞生长因子的生长而增加断端血液供应、促进骨愈合,同时该技术可激发骨形态结合蛋白,诱导骨生长。此外,手风琴技术还会激活成骨相关信号通路,来进一步促进骨的生长愈合。故本文拟将手风琴技术治疗骨不愈合的分子机制做一综述,以为该技术在骨不愈合方面的临床研究提供借鉴参考。Bone nonunion is one of the common and intractable complications in orthopedics.It not only prolongs the treatment,increases the burden on the family and society,but even induces severe consequences such as deformity and disability.However,accordion technology(AT)based on distraction osteogenesis has a significant role in the treatment of bone nonunion.It has been found that the traction stress,pressure and axial micromotion stimulate release of vascular endothelial growth factor,increase the blood supply at the bone end to promote bone healing,additionally activate bone morphogenic protein to induce bone growth.Furthermore,AT also activate osteogenesis related signaling pathways to further promote bone growth and healing.Therefore,this paper will review the molecular mechanism of AT in the treatment of bone nonunion,in order to provide a reference for the treatment of bone nonunion.
