摘  要:[目的]采用有限元方法研究距骨骨软骨损伤(OLT)的生物力学特点。[方法]将HeppleⅠ-Ⅴ期OLT患者和健康人足踝部的CT和MRI扫描数据导入三维建模软件,并建立踝关节三维有限元模型和进行有限元网格划分。测量相关力学参数。[结果]正常组胫骨旋转角度显著高于HeppleⅠ-Ⅴ期OLT各组(P<0.05);随着Hepple分期级别的增加OLT各组胫骨旋转角度显著减少(P<0.05)。相反,正常组胫骨关节面接触压力显著小于HeppleⅠ-Ⅴ期OLT各组(P<0.05);随着Hepple分期级别的增加OLT各组胫骨关节面接触压力显著增加(P<0.05)。此外,正常组距骨关节面最大压应力均显著低于HeppleⅠ-Ⅴ期OLT各组(P<0.05);随着Hepple分期级别的增加,OLT各组距骨关节面最大压应力显著增加(P<0.05)。[结论]随骨软骨损害程度加重,关节最大压应力显著增加。[Objective] To study biomechanical characteristics of osteochondral lesion of the talus(OLT) by finite element techniques.[Methods] The CT and MR scanning data of ankle and foot were obtained from patients who suffered from Hepple stage Ⅰ~Ⅴ OLT,and the healthy persons.After processed with the 3 D modeling software,the 3 D finite element models of anklet were established with division of finite element mesh.The biomechanical parameters,such as tibial rotation angle of ankle,contact pressure of tibial articular surface and maximal compressive stress on the talus,were recorded by simulating the standing state.[Results] The tibial rotation angle of ankle proved significantly greater in the normal control group than those in the OLT groups of Hepple stage Ⅰ~Ⅴ(P<0.05),and decreased significantly as upgrading Hepple stages in the OLT groups(P<0.05).On the contrary,the contact pressure of tibial articular surface was significantly less in the normal control group than those in the OLT groups of Hepple stage Ⅰ~Ⅴ(P<0.05),and significantly rose as advancing Hepple stages in the TOL groups(P<0.05).In addition,the maximal compressive stress on the talus surface also proved significantly less in the normal control group than those in the TOL groups of Hepple stage Ⅰ~Ⅴ(P<0.05),and significantly increased as advancing Hepple stages in the TOL groups(P<0.05).[Conclusion] The three-dimensional geometric model of OLT with high accuracy does be established by using the finite element techniques.As progression of OLT,the stress on the joint considerably increase.
