国医大师张学文辨治难治性高血压思路与经验

摘　　要：介绍国医大师张学文教授辨证论治难治性高血压的思路与临证经验。张学文教授辨识现代难治性疾病善于抓住该病的共性症状,“从症辨证”与“从病辨证”相结合,从而把握该病的基本病机。“血压高”这一体征贯穿难治性高血压病程始终,故将该体征作为一个共性症状纳入中医阴阳辨证理论体系进行辨证,并结合现代医学对难治性高血压病因病机的认识阐述了“阳亢”是难治性高血压病的基本病机,痰、湿、瘀是该病的重要兼证,本虚是其基本病性。基于辨证,提出了相应的治则治法,其中“治阳亢”是治疗难治性高血压病的核心。同时结合常用药及经验方,介绍了张学文教授对不同证型难治性高血压病的用药经验。This paper introduces the thinking and clinical experience of professor ZHANG Xue-wen,a master of Chinese medicine,in the treatment of refractory hypertension(RH)based on syndrome differentiation.It is believed that the understanding of modern refractory diseases should be good at grasping the common symptoms of the disease,‘syndrome differentiation based on symptoms’and‘syndrome differentiation based on disease’,so as to grasp the basic pathogenesis of the disease.Hypertension is a sign that runs through the course of RH.It should be integrated into the theory system of yin-yang syndrome differentiation of TCM as a common symptom to differentiate the symptoms.Combining with modern medicine’s understanding of the etiology and pathogenesis of RH,this paper expounds that‘yang hyperactivity’is the basic pathogenesis of RH.Phlegm,dampness and blood stasis are important symptoms of RH.Deficiency is the basic pathological nature of RH.Based on differentiation of symptoms and signs,the corresponding treatment principles and methods are put forward,in which‘treating yang hyperactivity’is the core of the treatment of RH.At the same time,combined with the commonly used drugs and experience prescriptions,the experience of professor ZHANG Xue-wen in the treatment of RH of different syndromes was introduced.
