三黄消疕膏治疗寻常型银屑病血热证临床疗效及对外周血TNF-α、IL-17的影响

摘　　要：目的:探讨三黄消疕膏治疗寻常型银屑病血热证患者的临床疗效及对外周血肿瘤坏死因子-α(TNF-α)、白细胞介素-17(IL-17)含量的影响。方法:将68例寻常型银屑病血热证患者随机分为对照组与治疗组,每组34例,对照组每晚外用卡泊三醇软膏,治疗组每晚外用三黄消疕膏,两组均内服决银颗粒,每次1袋,每日2次。治疗8周后比较两组患者治疗前后皮损面积及严重程度指数(PASI评分),评价临床疗效并检测外周血TNF-α、IL-17含量。结果:治疗组治疗后PASI评分显著低于对照组(P<0.01);治疗组显效率为87.87%(29/33),优于对照组的56.25%(18/32)(P<0.05);两组治疗后TNF-α、IL-17含量均显著低于治疗前(P<0.01),且治疗组低于对照组(P<0.05)。结论:三黄消疕膏能有效降低寻常型银屑病血热证患者PASI评分及血清TNF-α、IL-17含量,临床疗效确切。Objective:To investigate the clinical efficacy of Sanhuang Xiaobi Ointment in the treatment of psoriasis vulgaris with blood-heat syndrome and its influence on the levels of TNF-α and IL-17 in peripheral blood.Methods:Sixty-eight patients of psoriasis vulgaris with blood-heat syndrome were randomly divided into the control group and treatment group,34 cases in each group.The control group was treated with Captopril Ointment and the treatment group was treated with Sanhuang Xiaobi Ointment every night.Both groups were treated with Jueyin Granule internally,one bag every time,two times a day.After 8 weeks of treatment,the area and severity index(PASI score)of skin lesions in the two groups were compared before and after treatment.The clinical efficacy was evaluated and the contents of TNF-α and IL-17 in peripheral blood were measured.Results:After treatment,the PASI score of the treatment group was significantly lower than that of the control group(P<0.01);the effective rate of the treatment group(87.87%,29/33)was better than that of the control group(56.25%,18/32)(P<0.05);the contents of TNF-αand IL-17 in the two groups were significantly lower than those before treatment(P<0.01),and the treatment group was significantly lower than the control group(P<0.05).Conclusion:Sanhuang Xiaobi Ointment can effectively reduce PASI score and serum TNF-α and IL-17 contents in patients with psoriasis vulgaris with the blood-heat syndrome.The clinical effect is definite.
