从“痹”论治产科抗磷脂抗体综合征

摘　　要：产科抗磷脂抗体综合征(APS)为正虚标实之证,发病以肾虚为始动因素,分肺肾气虚、脾肾两虚、正虚血瘀、气衰血瘀四期进展。针对其正虚标实的总病机与疾病进展、证候表现特点,提出从"痹"论治的概念。二者均为正虚血瘀之证。产科APS病主要发生在血脉,以脉痹为基础演变,表现为正虚为本,在病情进展过程中正虚与瘀血的程度逐渐加重。具体原则为急则治标,缓则扶正,初益肺肾,再以健脾,久则化瘀,注重孕前孕期同治,再根据各期症状临证加减。As healthy qi deficiency and pathogen excessiveness,initiating factors of obstetric anti-phospholipid antibody syndrome is kidney-deficiency.Anti-phospholipid antibody syndrome could be a experience of four:lung and kidney qi deficiency,spleen and kidney deficiency,healthy qi deficiency and blood stasis,qi weakness and blood stasis.Aiming at the characteristics of the total pathogenesis,progression and syndrome manifestation of obstetric anti-phospholipid antibody syndrome,the concept of treatment from the aspect of‘Bi’is proposed.Both of them are asthenia in healthy qi and asthenia in superficiality.The obstetric anti-phospholipid antibody syndrome mainly occurs in the blood vessels,and it evolves on the basis of pulse bi.It is characterized by healthy qi deficiency,and qi deficiency and blood stasis gradually aggravates during the progress of the disease.The specific principle is to treat the emergent symptoms urgently,strengthen the body resistance slowly,improve the lungs and kidneys at first and then strengthen the spleen as well,dissipation blood stasis at last in long course.Modify prescription according to the clinical manifestations of each period.
