小针刀治疗耻骨直肠肌痉挛性便秘的临床疗效观察

摘　　要：目的:观察小针刀治疗耻骨直肠肌痉挛性便秘的临床疗效。方法:选择2016年10月至2018年10月就诊于中国人民解放军总医院中医肛肠科和北京马应龙长青肛肠医院的耻骨直肠肌痉挛性便秘患者共75例,按随机数字表法分为对照组38例、治疗组37例,对照组采用挂线法,治疗组采用小针刀切开缝合法。观察两组临床疗效,术前及术后1、3、6、12个月进行Wexner评分,术后1、3、6、12个月随访复发率。结果:治疗后两组临床疗效差异无统计学意义。术后6、12个月Wexner评分两组比较差异有统计学意义(P<0.01),术后6、12个月复发率治疗组低于对照组(P<0.01)。结论:小针刀治疗耻骨直肠痉挛性便秘,临床效果显著,为临床治疗耻骨直肠痉挛性便秘,提供了新思路和方法。Objective:To observe the clinical effect of treating constipation of puborectal muscle syndrome with small needle knife.Methods:A total of 75 patients with puborectal muscle syndrome who met the inclusion criteria from TCM Proctology Department of The General Hospital of the People’s Liberation Army and Beijing Mayinglong Combine Traditional Medicine Anorectal Hopital from October 2016 to October 2018 were selected,38 in the control group and 37 in the treatment group according to the random number table.The thread hanging method was used in the control group,and the small needle knife cutting and slotting method was used in the treatment group.The clinical effects in two groups were observed,the Wexner was scored before the operation and 1,3,6 and 12 months after the operation,recurrence rate was observed 1,3,6 and 12 months after the operation.Results:There was no significant difference between two groups.There were difference between the two groups at 6 and 12 months after operation in Wexner score(P<0.01).The recurrence rate in the treatment group was lower than that of the control group at 6 and 12 months after operation(P<0.01).Conclusion:The clinical effect of small needle scalpel in the treatment of puborectal spasmodic constipation is significant,which provides a new idea and method for the clinical treatment of puborectal spasmodic constipation.
