锁定钢板内固定加植骨术与钢板内固定术治疗跟骨骨折的价值

摘　　要：目的:研究锁定钢板内固定加植骨术与钢板内固定术治疗跟骨骨折的价值。方法:选取2016年2月-2018年2月于笔者所在医院就诊的跟骨关节内骨折患者60例,按照随机数字表法分为对照组(30例)和研究组(30例)。对照组行普通钢板内固定治疗,研究组行锁定钢板内固定加植骨治疗。观察两组患者临床疗效、骨解剖结构、足踝关节功能、恢复时间。结果:研究组临床有效率93.33%、AOFAS优良率93.33%均显著高于对照组的70.00%、66.67%(P<0.05)。治疗后两组患者Bohler角均显著升高(P<0.05),且研究组高于对照组(P<0.05);两组患者Gissane角均显著下降(P<0.05),且研究组显著低于对照组(P<0.05)。研究组住院时间短于对照组,愈合时间显著早于对照组(P<0.05)。结论:锁定钢板内固定加植骨治疗跟骨关节内骨折能显著提高临床效果,改善足踝关节功能,恢复骨解剖结构,缩短恢复时间,可临床推广。Objective:To study the effect of two kinds of internal fixation for calcaneal intra-articular fracture and the effect on the excellent rate of AOFAS after surgery.Method:A total of 60 patients with calcaneal intra-articular fracture who were treated in our hospital from February 2016 to February 2018 were selected and divided into control group(30 cases)and study group(30 cases)according to random number table method.The control group was treated with ordinary plate internal fixation,while the study group was treated with locking plate internal fixation plus bone grafting.The clinical efficacy,bone anatomy,ankle function and recovery time of the two groups were observed.Result:The clinical effective rate(93.33%)and the excellent and good rate of AOFAS(93.33%)in the study group were significantly higher than 70.00%,66.67%in the control group(P<0.05).After treatment,the Bohler angle increased significantly in both groups(P<0.05),and was higher in the study group than than in the control group(P<0.05).The Gissane angle decreased significantly in both groups(P<0.05),and which was significantly lower in the study group than that in the control group(P<0.05).The length of hospital stay was shorter,and healing time in the study group was significantly earlier than those in the control group(P<0.05).Conclusion:Locking plate internal fixation plus bone grafting in the treatment of intra-articular calcaneal fractures can significantly improve the clinical efficacy,improve ankle joint function,restore bone anatomy,shorten the recovery time,and can be clinically popularized.
