摘　　要：目的:分析心肌致密化不全患者的临床特征,为疾病诊断提供更多特征性依据。方法:采用回顾性分析法对石河子大学医学院第一附属医院明确诊断为心肌致密化不全的14例患者进行分析,对临床表现、心电图、超声心动图、血清肌钙蛋白T、NTpro-BNP等指标进行分析和总结。结果:14例心肌致密化不全患者的临床表现主要为胸闷、心悸、气促、阵发性呼吸困难、夜间端坐呼吸;NYHA分级多为Ⅲ级;心电图显示心律失常6例;超声心动图显示累及心尖部14例(100%),合并左心室和后壁病变6例(42.9%),累及两个或两个以上区域7例(50.0%)。NT-proBNP水平均升高;血清肌钙蛋白T升高3例(21.4%);D-二聚体升高11例(78.6%)。结论:心肌致密化不全可发生于各年龄段,且以男性较多见。临床表现以心衰症状为主,可合并多种心律失常。超声心动图为确诊方法,可见左心特异性改变。Objective:To analyze the clinical characteristics of patients with non-compaction of ventricular myocardium and provide more characteristic evidence for the diagnosis.Method:Retrospective analysis was conducted on 14 patients with non-compaction of ventricular myocardium in First Affiliated Hospital,School of Medicine,Shihezi University.Clinical manifestations,electrocardiogram,echocardiography,serum troponin T,NTpro-BNP and other indicators were analyzed and summarized.Result:The clinical manifestations of 14 cases with non-compaction of ventricular myocardium were chest tightness,palpitation,shortness of breath,paroxysmal dyspnea,and orthopnoea at night.NYHA classification was mostly gradeⅢ.The electrocardiogram indicated that arrhythmia in 6 cases.The echocardiography indicated that 14 cases(100%)involved the apex of the heart,and 6 cases(42.9%)with left ventricular and posterior wall lesions,and two or more regions were involved in 7 cases(50.0%).The level of NT-proBNP increased in 14 cases.Serum troponin T increased in 3 cases(21.4%),and D-dimer increased in 11 cases(78.6%).Conclusion:Non-compaction of ventricular myocardium can occur at all ages,and it is more common in men.The clinical manifestations are mainly heart failure,which could be combined with a variety of arrhythmias.Echocardiography is the diagnostic method,and the specific change of the left heart is observed.
