不同综合性医院请精神科会诊差异分析  

Analysis on the difference of consultation with psychiatry department in different general hospitals

摘　　要：目的:对比设有精神科综合性医院与无精神科综合性医院请精神科会诊的差异,为提升综合性医院精神卫生服务能力提供理论依据和数据支撑。方法:回顾性分析对比2018年天津市港口医院(有精神科)与天津市滨海新区某综合三甲医院(无精神科)邀请精神科会诊记录。结果:2018年有精神科综合医院精神科会诊127例,会诊率1.21%,无精神科综合医院精神科会诊共107例,会诊率0.36%;有精神科综合性医院请精神科会诊率远高于无精神科综合性医院(χ^2=94.96,P<0.01),有精神科综合性医院请精神科会诊理由以焦虑抑郁状态(28.35%)、睡眠障碍(22.05%)等精神问题为主,无精神科综合性医院请精神科会诊理由以兴奋躁动等精神问题(25.23%)、焦虑抑郁状态(16.82%)为主;非精神科医师识别精神心理问题能力不足。结论:综合性医院对精神卫生服务需求较大,无精神科综合医院开设精神卫生相关科室需加速,非精神科医师识别精神心理问题能力有待加强。Objective:To compare the differences of psychiatric consultation between general hospitals with psychiatry and general hospitals without psychiatry,so as to provide theoretical basis and data support for improving the capacity of mental health services in general hospitals.Methods:We retrospectively analyzed and compared the consultation records of invitation psychiatric department between Tianjin Port Hospital(with psychiatric department)and a comprehensive third-class hospital(without psychiatric department)in Binhai New District of Tianjin in 2018.Results:In 2018,there were 127 psychiatric consultation cases in general hospital with psychiatry,the consultation rate was 1.21%,and 107 psychiatric consultation cases in hospital without psychiatry,the consultation rate was 0.36%.The consultation rate of general hospital with psychiatry was much higher than that without psychiatry(χ^2=94.96,P<0.01).The main reasons for consultation were anxiety and depression(28.35%)and sleep disorder(22.05%)in general hospital with psychiatry.The main reasons for consultation were excitement and agitation(25.23%)and anxiety and depression(16.82%)in hospitals without psychiatry.The ability of non-psychiatrists to identify mental and psychological problems was insufficient.Conclusion:There is a great demand for mental health services in general hospitals.It is necessary to speed up the establishment of mental health related departments in general hospitals without psychiatry,and the ability of non-psychiatrists to identify mental and psychological problems needs to be strengthened.
