摘　　要：目的:探究腹腔镜联合胆道镜(LCBDE)治疗胆囊结石并肝外胆管结石的临床效果。方法:选取2017年2月-2019年5月于笔者所在医院就诊的胆囊结石并肝外胆管结石患者405例,按照随机数字表法分为对照组(202例)和观察组(203例)。对照组行传统开腹手术治疗,观察组行腹腔镜联合胆道镜治疗,观察两组临床疗效、血清IL-1β、TNF-α和TBIL水平及并发症情况。结果:观察组临床总有效率(100%)高于对照组(96.04%),差异有统计学意义(P<0.05)。治疗后观察组IL-1β、TNF-α和TBIL水平均低于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率(2.16%)低于对照组(10.00%),差异有统计学意义(P<0.05)。结论:LCBDE治疗胆囊结石并肝外胆管结石的临床效果显著,安全性良好,可有效降低患者体内的炎症水平。Objective:To explore the clinical effect of laparoscopy combined with choledochoscope(LCBDE)in treatment of cholecystolithiasis with extrahepatic bile duct stones.Method:From February 2017 to May 2019,405 patients with gallstones and extrahepatic bile duct stones were selected and divided into the control group(202 cases)and the observation group(203 cases).The control group was treated with traditional open surgery,the observation group was treated with laparoscopy combined with choledochoscopy,and the clinical effect,serum IL-1β,TNF-αand TBIL levels and complications of the two groups were observed.Result:The total clinical response rate in the observation group(100%)was higher than that in the control group(96.04%),the difference was statistically significant(P<0.05).After treatment,the levels of IL-1β,TNF-αand TBIL in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The incidence of complications in the study group(2.16%)was lower than that in the control group(10.00%),the difference was statistically significant(P<0.05).Conclusion:LCBDE is effective and safe in treatment of cholecystolithiasis and extrahepatic bile duct stones.It can effectively reduce the level of inflammation in patients.
