摘　　要：目的:探究老年股骨粗隆间骨折患者采用B超引导下腰丛复合坐骨神经阻滞麻醉的临床效果。方法:选取2017年2月-2019年10月笔者所在医院69例老年股骨粗隆间骨折患者。根据麻醉方法不同将其分为对照组(n=34)和试验组(n=35)。对照组采用蛛网膜下腔阻滞麻醉复合硬膜外麻醉,试验组采用B超引导下腰丛复合坐骨神经阻滞麻醉。比较两组麻醉效果及对免疫功能的影响。结果:试验组神经阻滞起效时间、维持时间均优于对照组,差异有统计学意义(P<0.05)。试验组术后不同时间点T细胞亚群指标均优于对照组,差异有统计学意义(P<0.05)。试验组术后不同时间点免疫球蛋白水平均优于对照组,差异有统计学意义(P<0.05)。结论:将B超引导下腰丛复合坐骨神经阻滞麻醉应用于老年股骨粗隆间骨折患者中,麻醉效果理想,对免疫功能的影响小,值得进一步推广。Objective:To investigate the clinical effect of B-ultrasound guided lumbar plexus combined with sciatic nerve block anesthesia in elderly patients with intertrochanteric fractures.Method:From February 2017 to October 2019,69 elderly patients with intertrochanteric fractures in our hospital were selected.According to different anesthesia methods,they were divided into the control group(n=34)and the experimental group(n=35).The control group underwent subarachnoid block anesthesia combined with epidural anesthesia.The experimental group was given B-ultrasound guided lumbar plexus combined with sciatic nerve block anesthesia.The anesthesia effects and its impact on immune function were compared between the two groups.Result:The onset time and maintenance time of nerve block in the experimental group were better than those in the control group,the differences were statistically significant(P<0.05).The indexes of T cell subsets in the experimental group were better than those in the control group at different time points after operation,the difference was statistically significant(P<0.05).The level of immunoglobulin in the experimental group were better than those in the control group at different time points after operation,the difference were statistically significant(P<0.05).Conclusion:B-ultrasound guided lumbar plexus combined with sciatic nerve block anesthesia is applied to elderly patients with intertrochanteric fractures.The anesthetic effect is ideal and has little impact on immune function,which is worth further promotion.
