摘  要:目的探讨18F-氟代脱氧葡萄糖正电子发射计算机断层显像/计算机体层扫描(18F-FDG PET/CT)和核磁共振成像(MRI)评估颈动脉粥样硬化(CAS)斑块炎症活性程度的价值。方法选取2017年3月至2018年3月上海交通大学医学院附属第九人民医院收治的38例CAS斑块患者为研究对象,均行18F-FDG PET/CT和MRI检查,根据斑块成分分为胶原斑块、脂质斑块、钙化斑块和出血斑块,比较四组靶-本底比值(TBR),并对各斑块MRI影像学结果进行分析。结果 38例患者中,CAS斑块仅累及左侧颈动脉6例,仅累及右侧颈动脉4例,累及双侧颈动脉28例;出血斑块、脂质斑块的TBR值明显大于钙化斑块、胶原斑块(P<0.05);出血斑块与脂质斑块的TBR值比较,差异无统计学意义(P>0.05);钙化斑块与胶原斑块的TBR值比较,差异无统计学意义(P>0.05)。结论 18F-FDG PET/CT和MRI检查可从斑块形态、成分、信号特点等方面来反映CAS斑块炎症活动程度,对预测CAS斑块患者血栓形成事件具有潜在的应用价值。Objective To explore the value of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) and magnetic resonance imaging (MRI) in assessing the plaque inflammatory activity degree of carotid atherosclerosis (CAS).Methods A total of 38 patients with CAS plaque admitted to cardiovascular department of our hospital from March 2017 to March 2018 were selected for the study.All patients were given 18F-FDG PET/CT and MRI.They were divided into collagen plaque,lipid plaque,calcified plaque and hemorrhagic plaque according to the plaques component.The target-to-background ratios (TBR) were compared among the four groups,and MRI imaging results of each plaque were analyzed.Results Among the 38 patients,in CAS plaques,there were only 6 cases of involvement of left carotid artery,only 4 cases of involvement of right carotid artery and 28 cases of involvement of bilateral carotid arteries.The TBR value of hemorrhagic plaque and lipid plaque was significantly larger than that of calcified plaque and collagen plaque (P<0.05).There was no significant difference in TBR value between hemorrhagic plaques and lipid plaque (P>0.05).There was no statistical difference in TBR value between calcified plaques and collagen plaque (P>0.05).Conclusion 18F-FDG PET/CT and MRI can reflect the degree of CAS plaque inflammatory activity from the aspects of plaque morphology,composition and signal characteristics,and it has potential application value in predicting thrombotic events in patients with CAS plaque.
