摘 要:目的探讨甲状腺功能正常的妊娠糖尿病(GDM)患者甲状腺激素水平与糖脂代谢的关系。方法选取妊娠24周~28周GDM患者150例,另选取健康体检孕妇126例作为对照组,收集其空腹血样本用于血糖、血脂、HbA(1c)、甲状腺激素水平等检测。结果与对照组相比,GDM患者孕前BMI、LDL-C、TC、TG、FBG和HbA(1c)升高,而HDL-C、FT3和FT4降低(P <0.05)而2组之间TSH差异无统计学意义(P>0.05);Pearson相关分析提示在GDM患者中,FT3与LDL-C、TC、TG、FBG和HbA1c呈负相关(r值分别为-0.161、-0.165、-0.194、-0.189、-0.183,P<0.05);多元逐步回归分析提示FT3与TG(β=-0.182,P <0.05)和FBG(β=-0.174,P<0.05)密切相关;FT4与LDL-C、TTC、TG、FBG和HbA(1c)呈负相关(r值分别为-0.176、-0.181、-0.202、-0.196、-0.185,P<0.05);多元逐步回归分析提示FT4与TG(β=-0.193 P<0.01)、FBG(β=-0.189,P<0.01)和HbA(1c)(β-0.178,P<0.05)密切相关;而TSH与HDL-C、LDL-C、TC、TG、FBG和HbA(1c)均无明显相关性(P>0.05)。结论在GDM患者中FT3和FT4与糖脂代谢密切相关。Objective To explore the correlation of serum thyroid hormones with glucose and lipid metabolism in pregnant women with gestational diabetes(GDM) whose thyroid functions with normal.Methods Totally 150 GDM and 126 healthy pregnant women were enrolled in our study.Their blood glucose,lipid,HbAlc and thyroid functions were determined.Results Compared with control group,patients with GDM had higher progestation BMI,LDL-C,TC,TG,FBG and HbA1 cwhile lower HDL-C,FT4 and FT3,and there was no statistical significance on the difference of TSH between two groups(P <0.05).In pregnant women with GDM,FT3 was negatively correlated with LDL-C,TC,TG,FBG and HbA1 c(r=-0.161,r=-0.165,r=-0.194,r=-0.189,r=-0.183,P <0.05).In multivariate regression analysis,FT3 was significantly correlated with TG(β=-0.182,P <0.05) and FBG(β=-0.174,P <0.05).FT4 was negatively correlated with LDL-C,TC,TG,FBG and HbA1 c(r=-0.176,r=-0.181,r=-0.202,r=-0.196,r=-0.185,P <0.05).In multivariate regression analysis,FT4 was significantly correlated with TG(β=-0.193,P <0.01),FBG(β=-0.189,P <0.01) and HbA1 c(β=-0.178,P <0.05).But there were no correlation of TSH with glucose and lipid metabolism(P>0.05).Conclusion FT3 and FT4 are significantly correlated with glucose and lipid metabolism in pregnant women with GDM.
