摘  要:目的了解余杭地区腹泻患者中致泻性大肠埃希菌(DEC)的流行状况和病原特征。方法对2014年-2016年哨点监测医院腹泻患者粪便标本进行分离培养,采用实时荧光PCR检测毒力基因,使用微量肉汤稀释法进行药物敏感试验,采用多位点序列分型(MLST分型)分析EAEC的基因同源性。结果 1 490份粪便标本中分离出149株DEC,阳性率为10.0%,以EAEC(68.5%)和ETEC(22.1%)为主;DEC感染率在性别和年龄上无差异,具有明显的季节性分布特征;16种抗生素中氨苄西林的耐药率最高,为59.73%,而阿米卡星、呋喃妥因的敏感率达100.00%;多重耐药株占19.46%,产ESBL菌株的检出率为26.17%。102株EAEC分离株中发现65种不同的ST型。结论余杭地区腹泻患者DEC的感染类型以EAEC和ETEC为主,存在季节分布特征;EAEC的ST型较分散,关联度较小,应关注EAEC暴发的风险。Objective To understand the epidemiologic and etiologic characteristics of diarrheagenic Escherichia coli(DEC)infections in Yuhang District.Methods Stool samples from diarrheal patients were cultured which were collected from sentinel hospitals during 2014-2016.Virulence genes were examined by real-time PCR.Drug sensitivity test was performed by broth microdilution method.The genetic homology of enteroaggregative E.coli(EAEC)was analyzed by multilocus sequence typing(MLST typing).Results A total of 149 DEC strains were isolated from l 490 stool samples(10.0%).EAEC and ETEC were predominant(68.5%and 22.1%);DEC infection rate had no difference by sex or age and had obvious seasonal distribution characteristics.Among the 16 antibiotics,the resistance rate of ampicillin was 59.73%,while the sensitivity rate of amikacin and furantoin were 100.00%.Multidrug-resistant strains accounted for 19.46%.The detection rate of ESBL strain was 26.17%.65 different ST types were found in 102 EAEC isolates.Conclusion EAEC and ETEC are predominant in DEC strains isolated from diarrheal patients in Yuhang.Season distribution characteristics of DEC infections are observed.ST types of EAEC are more dispersed and less relevance,so close attention should be paid to the possible EAEC outbreak.
