摘　　要：目的:应用儿童疼痛行为量表(FLACC)比较腹腔镜与传统疝囊高位结扎术治疗小儿腹股沟斜疝的术后疼痛。方法:选择2018年1月至12月收治的60例腹股沟斜疝患儿,患儿10个月~5岁2个月,按照随机数字表法将患者分为两组,观察组(n=30)采用双钩疝钩针法行腹腔镜疝囊高位结扎术,对照组(n=30)行传统开放经腹股沟管疝囊高位结扎术,分别于麻醉清醒后4 h(T1)、8 h(T2)、24 h(T3)应用FLACC进行疼痛程度的评估。结果:T1、T2、T3时,观察组FLACC评分为(5.73±1.29)、(3.20±1.27)、(1.50±0.73),对照组为(6.43±0.97)、(3.80±0.93)、(1.93±0.79),各时点观察组疼痛程度轻于对照组,差异有统计学意义(P<0.05)。结论:与传统手术相比,腹腔镜下疝囊高位结扎术治疗小儿腹股沟斜疝可有效减轻术后疼痛,创伤小,值得临床推广。Objective:To identify the differences in postoperative pain between laparoscopic and conventional high ligation of hernia sac in the treatment of pediatric indirect inguinal hernia with face legs activity cry consolability(FLACC).Methods:From Jan.2018 to Dec.2018,60 children were selected,they suffered from indirect inguinal hernia with the age from 10 months to 5 years and 2 months old.Patients were divided into two groups by random number table.30 children in observation group were treated with laparoscopic high ligation of hernia sac with double hernia needle,and 30 children in control group were treated with conventional operation.The FLACC was applied to assess the postoperative pain of the two groups at 4 h,8 h and 24 h after anesthetic awareness.Results:At 4 h,8 h and 24 h after anesthetic awareness,the FLACC scores of the observation group were(5.73±1.29),(3.20±1.27)and(1.50±0.73),respectively,while the FLACC scores of the corresponding time of the control group were(6.43±0.97),(3.80±0.93)and(1.93±0.79),respectively.The pain of children in observation group at each time point was lighter than that in the control group,there was significant difference between the two groups(P<0.05).Conclusions:Compared with conventional high ligation of hernia sac,laparoscopic treatment can effectively reduce postoperative pain of children who suffer from indirect inguinal hernia,with small trauma.It deserves to be recommended in the clinical application.
