摘　　要：目的:调查呼和浩特市冠心病患者延续性护理实施现状及影响因素,为冠心病患者建立有效延续护理模式提供科学依据。方法:采用问卷调查法,对内蒙古医科大学附属医院心血管内科住院200例冠心病患者进行相关调查。采用SPSS 20.0统计软件进行分析。结果:冠心病患者延续性护理实施现状不满意,主要影响因素是出院指导不全面、出院后续回访不到位以及缺少社区专业服务人员密切追踪。结论:冠心病患者延续性护理缺乏合理的实施措施与有效途径,建议完善医院对冠心病患者出院的全面指导护理与回访,加强社区的专业服务与日常指导。Objective To investigate the status quo and influencing factors of continuous nursing for patients with coronary heart disease in Hohhot,and to provide scientific basis for the establishment of effective continuing nursing model for patients with coronary heart disease.Methods A questionnaire survey was conducted on 200 patients with coronary heart disease in the Department of Cardiology,Affiliated Hospital of Inner Mongolia Medical University.SPSS 20.0 statistical software was used for analysis.Results The current situation of continuous care for patients with coronary heart disease is not satisfactory.The main influencing factors are incomplete discharge guidance,incomplete follow-up visit and lack of close follow-up by community professional service personnel.Conclusions There is a lack of reasonable implementation measures and effective approaches for continuous care of patients with coronary heart disease.It is suggested to improve the hospital's comprehensive guidance nursing and return visit for patients with coronary heart disease discharged from hospital,and strengthen the community's professional services and daily guidance.
