真皮中层弹性组织溶解1例

摘　　要：患者男,31岁,背部及枕部多发丘疹2年余,无自觉症状,缓慢增多。皮肤科情况:背部及枕部头皮可见多个散在分布的毛囊为中心的皮色扁平丘疹,长径约0.5 cm,丘疹上可见与皮纹一致的松弛性纹理。皮损组织病理示:表皮轻度萎缩,真皮中部胶原纤维变细,排列疏松,未见明显日光性纤维弹性变性。弹力纤维染色:真皮中部可见弹力纤维明显减少,断裂。诊断:真皮中层弹性组织溶解症。A 31-year-old male presented multiple papules on his back and occipital scalp for 2 years.The patient was asymptomatic and the lesions gradually developed.Physical examination revealed multiple isolated flat flesh papules around the follicle,which was about 0.5 cm in diameter.There were slack wrinkles on the lesions consistent with the direction of skin texture.Histopathology revealed mild atrophy of epidermis and the collagen fibers in the mid-dermis became thinner and loose,and no obvious elastic degeneration of solar fibers was observed.Verhoeff-Van Gieson stains showed mid-dermal loss and fracture of elastic tissue.The diagnosis was made as mid-dermal elastolysis(MDE).
