血清同型半胱氨酸,胱抑素C,超氧化物歧化酶对2型糖尿病肾病的早期诊断价值

摘　　要：目的联合血清同型半胱氨酸(Hcy)、胱抑素C(Cys C)、超氧化物歧化酶(SOD)三者对糖尿病肾病进行早期诊断。方法选取2015年5月-2019年4月间在我院就诊的糖尿病患者120例作为研究对象,并且根据尿白蛋白排出率在30-300mg/d分为糖尿病肾病组68例和<30mg/d单纯糖尿病组52例,同期选择健康体检者50例作为健康对照,所有受试者检测血清Hcy、Cys-C与SOD,分析三者对糖尿病的诊断效能及糖尿病肾病的早期诊断价值。结果Hcy、Cys-C、SOD诊断糖尿病的灵敏度分别为68.3%、66.7%、64.2%,特异度分别为86.0%、84.0%、88.0%,准确度分别为73.5%、71.8%、71.2%,并联诊断的灵敏度、特异度及准确度分别为80.0%、82.0%及80.6%;Hcy、CysC、SOD诊断糖尿病肾病的灵敏度分别为91.2%、92.6%、88.2%,特异度分别为61.5%、67.3%、67.3%,准确度分别为78.3%、81.7%、79.2%,串联诊断的灵敏度、特异度及准确度分别为85.3%、82.7%及84.2%。结论血清同型半胱氨酸、胱抑素C、超氧化物歧化酶的并联试验对于糖尿病有较好诊断价值,而其串联试验则更有助于糖尿病肾病的早期诊断。Objective Combined with serum homocysteine(Hcy),cystatin C(Cys C)and superoxide dismutase(SOD)for early diagnosis of diabetic nephropathy.Methods A total of 120 diabetic patients who were treated in our department from May 2015 to April 2019 were selected as the research object,and were divided into 68 cases of diabetic nephropathy group and<30 mg according to the urine albumin excretion rate of 30-300 mg/d./d 52 cases in the simple diabetes group,50 healthy subjects were selected as healthy controls during the same period.All subjects tested serum Hcy,Cys-C and SOD,and analyzed the diagnostic efficacy of the three for diabetes and the early diagnosis value of diabetic nephropathy.Results The sensitivity of Hcy,Cys-C and SOD in diagnosing diabetes was 68.3%,66.7% and64.2%,the specificity was 86.0%,84.0% and 88.0%,and the accuracy was 73.5%,71.8%and 71.2%respectively.The sensitivity,specificity and accuracy of diagnosis were 80.0%,82.0% and 80.6% respectively;the sensitivity of Hcy,Cys-C and SOD in diabetic nephropathy were 91.2%,92.6%and 88.2%,and the specificity was 61.5%,67.3%,67.3%,the accuracy is 78.3%,81.7%,79.2%,the sensitivity,specificity and accuracy of the tandem diagnosis are85.3%,82.7%and 84.2%,respectively.Conclusion The parallel test of serum homocysteine,cystatin C,and superoxide dismutase has a good diagnostic value for diabetes,and its serial test is more helpful for the early diagnosis of diabetic nephropathy.
