摘　　要：目的回顾分析皮肤恶性肿瘤术后复发病例的临床资料,总结规律。方法对2014年1月至2018年12月在昆明医科大学第二附属医院皮肤科行皮肤恶性肿瘤切除术后复发的30例病例的临床资料进行回顾分析。结果复发病例中老年人居多,平均年龄62.30岁,男性占比为53.33%,女性46.67%;复发病例中SCC和BCC最为常见,占比分别为33.33%及16.67%;复发部位多分布于鼻唇及鼻唇沟、近阴道、尿道等解剖结构特殊及皮肤粘膜交界处,其中位于头面部的最多,占全部病例的40.00%;复发病例中30.00%发生淋巴结转移,其中CMM复发时均已发生淋巴转移,发生死亡病例占比50.00%,预后差。结论皮肤恶性肿瘤术后复发后,误诊情况、恶性程度、侵袭性、转移情况增加,了解造成术后复发的可能原因及降低复发的手术策略十分必要。Objective To retrospectively analyze and summarize possible causes of recurrence of skin malignant tumor and to provide corresponding preventive measures to reduce the recurrence. Methods The clinical data of 30 cases of recurrence of skin malignant tumor resection in dermatology department of the 2 nd Affiliated Hospital of Kunming Medical University from January 2014 to December 2018 were retrospectively analyzed.Results Most of the recurrence cases happened to the elderly, with an average age of 62.30 years old, and men account for 53.33%, slightly more than women. SCC and BCC were the most common among all recurrence cases,accounting for 33.33% and 16.67%, respectively. Recurrent sites were mostly distributed in areas with special anatomical structures such as nose,lip, nasolabial groove, vagina and urethra, and the junction of skin and mucosa, among which the most usual location was face and head, accounting for 40.00% of all cases. Lymph node metastasis occurred in 30.00% of recurrence cases. Lymphatic metastasis occurred in all the CMM recurrence cases.Among the relapsed CMM cases,death accounted for 50.00%, showing a poor prognosis. Conclusion When postoperative skin malignant tumor recurred, situations such as misdiagnosis, deterioration, invasiveness and metastasis could increase. It is necessary to understand the possible causes of postoperative recurrence and provide reasonable surgical strategies to reduce recurrence.
