非内分泌科糖代谢异常患者管理情况调查研究

摘　　要：目的调查非内分泌科糖代谢异常患者血糖管理情况。方法选取2018年8—10月出院的非内分泌科监测过血糖的患者(4357例),根据血糖监测情况分为糖代谢正常和异常患者。调查糖代谢异常患者(708例)入院后空腹血糖、餐后血糖、糖化血红蛋白测定及会诊情况,出院时血糖是否达标,是否有血糖相关诊断,出院注意事项告知情况,再进行统计分析。结果糖代谢异常患者空腹血糖测定率63.84%,餐后血糖测定率76.13%,糖化测定率36.30%,请内分泌专科会诊率13.56%,出院血糖达标率35.45%,出院诊断率54.66%,出院告知率35.59%。内科在餐后血糖和糖化测定、出院诊断和告知的管理明显优于外科(P<0.05),会诊与未会诊患者血糖达标情况无明显差异(P<0.05)。结论非内分泌科医生对糖代谢异常患者血糖管理还有很大的提升空间,特别是应该强化外科医生的血糖管理意识,增加与内分泌科联系,共同提高血糖管理质量。Objective To investigate the glucose management of patients with abnormal glucose metabolism in non-endocrinology department.Methods Patients(4,357 patients)from non-endocrinology department who were discharged from of the hospital from August 1,2018 to October 31,2018 were selected and divided into patients with normal glucose metabolism and abnormal glucose metabolism according to the glucose monitoring.To investigate fasting blood glucose,postprandial blood glucose and HbA1 c measurement,consultation,as well as whether the blood glucose reached the standard,diagnosis and notice at discharge of patients with glucose metabolism abnormality(708 patients)after admission,and then conduct statistic analysis.Results For patients with abnormal glucose metabolism,the determination rate of fasting glucose was 63.84%,rate of postprandial glucose detection was 76.13%and it was 36.30%of HbA1 c,the consultation rate of endocrinology specialist was 13.56%,the rate of glucose reaching the criteria was 35.45%,the diagnosis rate of patients with abnormal glucose was 54.66%,and the notice rate of discharge was 35.59%.Significant differences between medical and surgical patients in measurement of postprandial blood glucose,HbA1 c,diagnosis and notice(P<0.05)were observed.However,there was no significant difference between consultation and non-consultation patients in blood glucose standard(P>0.05).Conclusions There is still much room for non-endocrinologists to improve glucose management in patients with abnormal glucose metabolism.In particular,the awareness of blood glucose management of surgeons should be strengthened,and the association with endocrinologists should be increased to jointly improve the quality of blood glucose management.
