摘 要:目的研究应用单唾液酸神经节苷脂治疗重度中枢神经受损患者后血清中神经元特异性烯醇化酶(NSE)、S100B蛋白、基质金属蛋白酶-9(MMP-9)表达水平及其临床意义。方法选取2018年1-11月该院收治的117例中枢神经损伤患者作为研究对象,随机分为观察组(58例)和对照组(59例)。两组患者均采用综合治疗法,观察组患者在综合治疗的基础上增加单唾液酸神经节苷脂注射液治疗。检测两组患者治疗前、治疗30 d后血清NSE、S100B蛋白、MMP-9水平,观察两组患者治疗前后格拉斯哥昏迷指数评分(GCS评分)、简易智力精神状态检查量表(MMSE)评分及治疗后的临床疗效。结果两组患者治疗30 d血清NSE、S100B蛋白、MMP-9表达水平均低于本组治疗前,且观察组均明显低于对照组,差异均有统计学意义(P<0.05)。两组患者治疗30 d后GCS评分高于本组治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05)。两组患者治疗30 d后MMSE评分均高于本组治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05)。观察组患者治疗30 d后临床总有效率为72.41%(42/58),高于对照组的55.93%(33/59),差异有统计学意义(P<0.05)。结论重度中枢神经损伤患者血清NSE、S100B蛋白及MMP-9水平呈高表达,单唾液酸神经节苷脂能降低血清NSE、S100B蛋白及MMP-9水平,有效治疗各种类型颅内出血、感染、脑梗死等引发的重度中枢神经损伤,但其作用效果有限,大多数患者无法达到完全康复。临床可根据患者具体情况,在常规治疗的基础上给予单唾液酸神经节苷脂注射治疗。Objective To investigate the effect of monosialoganglioside on serum levels of neuron specific enolase(NSE),S100B protein and matrix metalloproteinase-9(MMP-9)in patients with severe central nervous system injury.Methods A total of 117 patients with central nervous system injuries admitted to the hospital from January 2018 to November 2018 were selected,and were randomly divided into observation group(58 cases)and control group(59 cases).Both groups received the comprehensive therapy,based on this,the observation group received the injection therapy of monosialoganglioside.Serum levels of NSE,S100B protein and MMP-9 before treatment and on 30 days after treatment were measured,and the Glasgrow Coma Scale(GCS)score,Mini-mental State Examination(MMSE)score and clinical effect of the two groups after treatment were observed.Results After 30 days of treatment,the serum levels of NSE,S100B protein and MMP-9 were decreased in both groups(P<0.05),and were lower in the observation group than in the control groups,and the differences were statistically significant(P<0.05).GCS scores were increased in both groups on 30 days after treatment,and were higher in the observation group than in the control group,and the differences were statistically significant(P<0.05).MMSE scores on 30 days after treatment were increased in both groups,and were higher in the observation group than in the control group,and the differences were statistically significant(P<0.05).The clinical efficiency in the observation group on 30 days after treatment was 72.41%(42/58),which was higher than 55.93%(33/59)in the control group,and the differences were statistically significant(P<0.05).Conclusion The levels of NSE,S100B protein and MMP-9 in the serum of patients with severe central nervous injury are high.Monosialoganglioside can reduce the levels of NSE,S100B protein and MMP-9 in the serum.It can effectively treat the severe central nervous injury caused by various types of intracranial hemorrhage,infection and cerebral infarction,but its effect is
