利用信息技术持续改进医院感染漏报率的实践与体会

摘　　要：目的利用信息化手段监测院内感染,持续改进监测质量,降低院内感染漏报率。方法选择某医院院感监测系统2017年2月1日―2018年12月31日的2847例次院内感染患者信息,比较实施持续质量改进各阶段措施前后,院内感染漏报率的差异。结果经过第一阶段实施质量改进后,院内感染漏报率为23.84%,第二阶段实施惩罚措施后,漏报率降至7.37%,与第一阶段相比差异有统计学意义(χ2=55.59,p<0.001)。引进新的院内感染监测系统后,漏报率有所上升,经过第三阶段采取设置预警提醒措施,漏报率降至2.05%(χ2=31.94,p<0.001)。结论采取漏报惩罚并结合医生端增加预警提醒设置为主导的持续质量改进措施,能有效降低院内感染漏报率。Objective To monitor nosocomial infections with information technology,so that to continuously improve the quality of nosocomial infections monitoring,and reduce the missing report rate of nosocomial infections.Methods The information of 2847 patients with nosocomial infections from February 1st,2017 to December 31st,2018 in a nosocomial infection surveillance system was selected to compare the differences in the rate of missed reports of nosocomial infections before and after the implementation of continuous quality improvement measures.Results After the implementation of quality improvement in the first stage,the missing report rate of nosocomial infections was 23.84%.After the implementation of punitive measures in the second stage,the missing report rate dropped to 7.37%,which was significantly different compared to the first stage(χ2=55.59,p<0.001).After the introduction of a new nosocomial infection monitoring system,the missing report rate has increased.After the third stage of taking early warning reminders,the missing report rate dropped to 2.05%(χ2=31.94,p<0.001).Conclusion Continuous quality improvement measures whose mainly measure was punishment for omission of report combining with the early warning reminders for doctors can effectively reduce the rate of missed reports of nosocomial infections.
