通关利窍针刺法联合吞咽治疗仪治疗脑梗死后吞咽障碍的疗效观察

摘　　要：目的:探讨通关利窍针刺法联合吞咽治疗仪治疗脑梗死后吞咽障碍的疗效。方法:将118例脑梗死后吞咽障碍患者随机分为两组各59例,对照组接受吞咽治疗仪治疗,研究组接受通关利窍针刺法联合吞咽治疗仪治疗。治疗前后采用洼田饮水实验(WTDWT)对患者进行评估,同时检测血清脑源性神经营养因子(BDNF)、血管内皮生长因子(VEGF)、胰岛素样生长因子-1(IGF-1)水平。结果:研究组治疗后的WTDWT评分为(3.27±0.79)分,明显高于对照组(2.39±0.51)分,研究组WTDWT评分提高明显高于对照组(P<0.05)。研究组治疗后的血清BDNF为(7.74±1.36)ng/mL、VEGF为(166.50±14.27)pg/mL、IGF-1为(160.83±29.17)pg/mL,明显高于对照组(5.74±1.08)ng/mL、(105.36±17.22)pg/mL、(113.27±23.28)pg/mL(P<0.05)。结论:吞咽治疗仪对脑梗死后吞咽障碍的效果有限,加用通关利窍针刺法治疗能够明显改善神经功能,提升效果。Objective:To explore the curative effect of Tongguan Liqiao acupuncture combined with acupuncture and swallowing therapy instrument on dysphagia after cerebral infarction.Methods:118 patients with dysphagia after cerebral infarction were randomly divided into two groups of 59 patients.The control group was treated with a swallowing therapy device,and the research group was treated with Tongguan Liqiao acupuncture combined with swallowing therapy device.Before and after treatment,Watian drinking water test(WTDWT)was used to evaluate patients,and serum levels of brain-derived neurotrophic factor(BDNF),vascular endothelial growth factor(VEGF),and insulin-like growth factor-1(IGF-1)were also detected.Results:The WTDWT score of the study group after treatment was(3.27±0.79),which was significantly higher than that of the control group(2.39±0.51),and the WTDWT score of the study group was significantly higher than that of the control group(P<0.05).The serum BDNF of the study group after treatment was(7.74±1.36)ng/mL,VEGF was(166.50±14.27)pg/mL,and IGF-1 was(160.83±29.17)pg/mL,which was significantly higher than that of the control group(5.74±1.08)Ng/mL,(105.36±17.22)pg/mL,(113.27±23.28)pg/mL(P<0.05).Conclusion:The swallowing therapy instrument has limited effect on dysphagia after cerebral infarction,and the addition of Tongguan Liqiao acupuncture treatment can significantly improve nerve function and enhance the effect.
