彩色多普勒超声联合超声造影及超声微血管成像技术在糖尿病患者穿支皮瓣动脉定位中的探讨研究

摘　　要：目的:探讨彩色多普勒超声联合超声造影及超声微血管成像技术在糖尿病患者穿支皮瓣动脉定位中的应用价值。方法:选择2017年12月~2018年12月在本院超声科检查的糖尿病患者(观察组),及健康检查者(对照组)各30例。比较两组穿支皮瓣血管内径、峰值流速、阻力指数、搏动指数、血流量及出皮点、出肌点、穿支与主干的关系的显示情况。结果:彩色多普勒超声显示观察组患者与对照组比较,峰值流速两组间有统计学差异;阻力指数、搏动两组间比较有统计学差异(P<0.05)。彩色多普勒超声、SMI、CEUS对出肌点、出皮点、穿支与主干的关系对照组的检出率高于观察组,有统计学差异(P<0.05),而SMI、CEUS对于两组的出肌点、出皮点、穿支与主干的关系的检出率优于彩色多普勒超声并有统计学差异(P<0.05)。结论:彩色多普勒超声联合超声造影及超声微血管成像对于糖尿病患者穿支皮瓣的显示更清晰、准确,且操作简单方便。Objective:To investigate the application value of color Doppler ultrasound combined with contrast-enhanced ultrasound and ultrasound microvascular imaging technology in the arterial localization of perforator flap in patients with diabetes mellitus.Methods:30 diabetic patients(observation group)and 30 health examiners(control group)were selected from December 2017 to December 2018 in our hospital.The vascular diameter,peak velocity,resistance index,pulsatility index,blood flow and the relationship between perforator and trunk were compared between the two groups.Results:Compared with the control group,the peak velocity of the observation group was significantly different from that of the control group;the resistance index and pulsation between the two groups were statistically different(P<0.05).The detection rate of color Doppler ultrasound,SMI,CEUS on the relationship between muscle outlet,skin outlet,perforator and trunk in the control group was higher than that in the observation group,with statistical difference(P<0.05),while the detection rate of the relationship between muscle outlet,skin outlet,perforator and trunk of the two groups by SMI and CEUS was better than that of color Doppler ultrasound with statistical difference(P<0.05).Conclusion:Color Doppler ultrasound combined with contrast-enhanced ultrasound and ultrasound microvascular imaging is more clear,accurate and easy to operate.
