摘  要:目的探讨弓形虫感染对精神分裂症患者临床诊疗的影响。方法选取100例首发精神分裂症患者,根据血清弓形虫IgG抗体水平分为弓形虫感染首发精神分裂症50例(观察组)和未感染弓形虫首发精神分裂症50例(对照组),采用简明精神病评定量表(BPRS)、阳性症状量表(SAPS)及阴性症状量表(SANS)比较两组患者的病情和精神症状,采用蒙特利尔认知评估量表(MoCA)比较两组的认知功能障碍,采用persnal相关性分析分析病人精神症状与认知功能障碍之间的关系。结果观察组BPRS、SAPS、SANS、MoCA总分分别为98.94±5.13、104.19±46.37、90.16±4.25和22.05±0.22,对照组分别为94.95±4.67、93.84±5.25、83.12±3.18、24.52±0.38。两组比较差异均有统计学意义(t值分别为7.12、8.86、9.36,9.38,均P<0.01)。相关性分析显示,BPRS、SAPS、SANS得分均与MoCA得分呈负相关(r值分别为-0.549、-0.628和-0.711,均P<0.01)。结论弓形虫感染能加重精神分裂症患者的病情,精神症状和认知功能障碍,精神分裂症病情和精神症状是认知功能的影响因素。Objectives To explore the impact of Toxoplasma infection on the treatment and diagnosis of schizophrenics. Methods Subjects were 100 patients with first-onset schizophrenia. All patients were divided into two groups according to the level of IgG antibodies against Toxoplasma gondii. Fifty patients with first-onset schizophrenia who were infected with Toxoplasma served as the observation group, and 50 schizophrenics who were not infected with Toxoplasma served as the control group. The Brief Psychiatric Rating Scale(BPRS), Scale for Assessment of Positive Symptoms(SAPS), and Scale for Assessment of Negative Symptoms(SANS) were used to assess the mental condition of schizophrenics. Cognitive impairment in the two groups was assessed and compared using the Montreal Cognitive Assessment(MoCA). The relationship between mental symptoms and cognitive impairment was analyzed using Pearson’s correlation analysis. Results The observation group’s total score on the BPRS was 98.94±5.13, its total score on the SAPS was 104.19±6.37, its total score on the SANS was 90.16±4.25, and its total score on the MoCA was 22.05±0.22. The control group’s total score on the BPRS was 94.95±4.67, its total score on the SAPS was 93.84.19±5.25, its total score on the SANS was 83.12±3.18, and its total score on the MoCA was 24.52±0.38, The total scores of the two groups differed significantly(T-values: 7.12, 8.86, 9.36, and 9.38, respectively, P<0.01 for all). Correlation analysis indicated that scores on the BPRS, SAPS, and SANS were inversely correlated with the score on the MoCA(r-values:-0.549,-0.628, and-0.711, respectively, P<0.01 for all). Conclusion A Toxoplasma infection may exacerbate the psychiatric symptoms and diminish the cognitive function of schizophrenics. Psychiatric symptoms were a factor influencing the cognitive function of schizophrenics.
