不同频率电针对脑卒中后吞咽障碍的干预作用

摘　　要：目的:在神经内科常规治疗,配合吞咽神经和肌肉电刺激仪治疗的基础上,比较不同频率电针对脑卒中后吞咽障碍的临床疗效差异。方法:将80例病例依据不同频率的电针治疗分为观察组和对照组,各40例。基础治疗无差异,针灸取穴采用局部取穴法,为廉泉穴、双侧夹廉泉穴,电针刺激均采用连续波,观察组给予1Hz频率、对照组给予5Hz频率,留针30min,1天1次,1个疗程6天,休息1天,共治疗4个疗程。总疗程结束后即评价其吞咽功能的改善情况,方法为洼田饮水试验。结果:电针治疗后,两组患者洼田饮水试验分级均较治疗前改善(均P<0.05),且观察组优于对照组(P<0.05),观察组总有效率为92.5%、对照组为62.5%,两组比较有统计学差异(P<0.05)。结论:1Hz频率电针对脑卒中后吞咽障碍疗效更佳。Objective:To compare the clinical effect of electroacupuncture at different frequencies on dysphagia after stroke on the basis of routine treatment in department of Neurology and electrostimulation of deglutition nerve and muscle.Methods:80 cases were divided into observation group and control group according to different frequency of electroacupuncture treatment,40 cases each.There was no difference in basic treatment.Local point selection was used in acupuncture and moxibustion.Lianquan and Jialianquan were used in both sides.Continuous wave was used in electroacupuncture.The observation group was given 1 Hz frequency and the control group was given 5 Hz frequency.The needles were kept for 30 min,once a day,one course of treatment for 6 days,and rest for 1 day,for a total of 4 courses of treatment.The improvement of swallowing function was evaluated after the whole course of treatment by water swallow test.Results:After electroacupuncture treatment,the grades of water swallow test in the two groups were improved(both P<0.05),and the observation group was better than the control group(P<0.05).The total effective rate of the observation group was 92.5%,and that of the control group was 62.5%,there was a statistical difference(P<0.05).Conclusion:The 1 Hz frequency electroacupuncture is more effective for dysphagia after stroke.
